EE WAS ATTACHING A HOSE TO THE WATER TRUCK. HE WAS STRUCK BY HOSE CAUSING HIM TO LOOSE HIS BALANCE AND FALL.

EE WAS GRINDING A 992 LOADER HIGHLIFT BUCKET WITH A 5900 RPM MILWAUKEE GRINDER WHEN THE GRINDING WHEEL CAUGHT ON A CUT IN THE BUCKET AND CAUSED THJE GRINDER TO KICK HITTING THE EE IN THE LEFT LEG JUST ABOVE THE KNEE.
EE WAS INSTALLING A WEAR PLATE ON THE CHUTE THAT FEEDS 3 & 4 CRUSHER. THE PLATE WAS 1" THICK MEASURING 14" X 18". THE PLATE WAS BEING HELD IN PLACE BY ANOTHER EE WHEN HE LOST CONTROL AND THE P LATE SLID DOWN ON INJURED EE'S LEFT INDEX FINGER.
