Kenvil Pit and Mill Metal/Non-Metal

Miner fell through catwalk and struck ground surface. Miner injured left ankle.

While removing the return roller from conveyor #11, the bolt on the bracket snapped causing the roller to fall on the left ring finger of the employee therefore causing a laceration to same.

A mechanic, working on piece of equipment in the shop. Worker was using parts cleaner to degrease a clutch. Worker then violated policy and procedure by using a hand-held butane torch. There was a flash from combustible material that caused burns to the face and head of the worker. Worker was conscious and talking. He was taken to the hospital by another worker was admitted.

While changing a screen on a portable crusher, the screen slipped down and pinched right pinky finger.

An elderly male, (non-employee) apparently suffering from early stages of alzheimer's disease and dementia, trespassed onto posted mine property. He most likely became disorientated and died of hypothermia. The pit was not in operation at the time.

Metal bar from tail pulley of conveyor belt fell onto finger, pinching finger between bar and metal frame.

While lifting/manuevering truck tire into place, employee strained shoulder.

Two plant operators attempting to remove plate steel from a cintron when the hook and cable slipped and the steel plate landed on EE's left hand and fractured left index finger.

RUNNING AIR GUN. TOOL SLIPPED INTO LEFT HAND THUMB AREA. LACERATION BETWEEN THUMB AND FIRST FINGER ON PALM AREA. RECEIVED 4 STITCHES AT HOSPITAL ER.

Claims hooking up a hose, pulled muscle in right shoulder and reported to supervisor for information only. No request or medical assistance requested. Company wants everything reported.
