Caught in, under or between a moving and a stationary object

Employee was part of team removing a screen to be taken to another quarry. When trying to remove screen it was still attached with a weld. Employee cut the weld and it became free and swung in employee's direction and pinched employee between the screen and the handrail. This caused injuries to employee's colon.

While employee went to open flop gate in the screen chute, gate created resistance, employee cleaned around gate, went to push gate down, lost balance and left and middle finger got caught between closed flop gate and wall of chute. Tip of middle finger was crushed resulting in tip of bone to have multiple chips and skin removed with exposure to muscle, fat and tip of bone.

while employee was in the process of fixing a torn belt, employee placed one hand under belt and drilled through belt and made contact with gloved hand creating a circular injury to right pointer finger resulting in 8 stitches

Fall from machine

While employee was bending over on the walkway of the excavator looking at oil sight glass, employee stood up and we think he felt dizzy and fell from walkway. Employee was taken to the hospital.
