A heavy duty centrifugal slurry pump was being installed. The pump was rigged with strap slung under the bearing frame of the pump. When the pump was lifted, it listed toward employee. Employee's right thumb was pinched between the strap and one of the motor mount support studs, causing a soft tissue injury to employee's right thumb.

The employee, was moving a suction line for the de-watering pump in the rain. The employee lost his footing due to the wet conditions. The employee strained his back.

Employees were attempting to thaw a frozen suction pipe. The pipe slipped and fell on employee's right index finger tip.

While cleaning side window of dump truck, employee slipped past two handrails and got caught on handrail resulting in a 3 cm laceration of right testicle.

A grating was leaned against a closed overhead door. The door was opened and the grating fell and hit the left knee of the worker resulting in superficial abrasion and tenderness. A strain resulted with no lost time.

Employee was lifting bags of dried product from one pallet and transferring them to an adjacent pallet and felt pain in lower back.

Employee was loading bulksach on pickup truck using fork-lift. A grain of sand blew into employee's eye. Employee rinsed eye but experienced discomfort following night.Doctor removed grain of sand and prescribed drops for irritation. Employee returned to work same day.
