I was showing an employee a proper stacking pattern for 50 lb bags on a pallet and as I was turning I felt a sharp pain and heard a popping sound in my right knee, also quite a bit of pain. EE - Plant Manager St. Cloud Mining Co. - Ash Meadows Plant p.s. I'm also the injured employee

EMPLOYEE WAS PUSHING AN OPEN HOPPER TO HELP IT DUMP WHEN HE OVEREXERTED HIMSELF AND AGGREVATED AN OLD INJURY (HERNIA). ORIGINAL INJURY OCCURRED APRIL 12, 2001 WHILE EMPLOYED AT IMV. PART OF BODY AFFECTED WAS THE RIGHT GROIN AREA. ASSOCIATE WORKED THE NEXT SCHEDULED SHIFTS THEN CALLED IN SORE MONDAY MORNING AND MISSED THE NEXT TWO SCHEDULED SHIFTS.

EE was using plastic pipe cleaner to prepare pipe for glue. EE wiped his brow with bare hand and continued working and continued to sweat. The sweat ran off his brow into his left eye causing a burn on the cornea.

EE FINISHED REBUILDING THE H20 PUMP & WAS READY TO REMOUNT TO THE H20 TRUCK. USING A HYDRAULIC BOTTOM JACK TWO WORKERS STARTED TO JACK UP THE PUMP IN PLACE. DURING THE ALIGNMENT OF MOUNTING HOLES THE PUMP FELL OFF JACK HITTING EE'S MIDDLE FINGER OF RIGHT HAND AGAINST THE FLOOD. EE WENT TO DOCTOR ON MONDAY 7/12/04.

AT ABOUT 7:30 AM EE CAME BY THE WINDOW TO MY OFFICE & KNOCKED. I WENT OUTSIDE TO SEE WHAT WAS NEEDED. EE STATED THAT HE HAD PINCHED HIS SCIATIC NERVE & THAT HE COULD NOT CLIMB THE STEPS. ASKED EE SPECIFICALLY OF OR WHAT HE DID TO HURT HIS BACK. HE REPLIED: NOTHING HE COULD DIRECTLY PLACE HIS FINGER ON. HE WAS GOING TO HIS CHIROPRACTOR.

EE USED A C-CLAMP (VISE-GRIP TYPE) TO CLAMP A DUST COLLECTOR SOCK TO AN ANGLE IRON & STARTED TO STRETCH THE SOCK. THE CLAMP CAME OFF & EE FELL BACKWARDS ONTO HIS BACK & HEAD. ORIGINALLY STATED HE WAS ALRIGHT, BUT A SHORT TIME LATER HE DID NOT KNOW WHERE HE WAS OR WHAT HE WAS DOING.

RECEIVED INCIDENT REPORT ON 9/10/02, RECEIVED DR.'S REPORT WITH RESTRICTIONS ON 9/26/02(LAST SCHEDULED SHIFT). NO REASONABLE ACCOMODATIONS CAN BE MADE, EE OFF DUTY TILL FULL RELEASE STAR

EMPLOYEE WAS OPERATING FORKLIFT. THERE WAS NO LOAD ON LIFT. HE WAS GOING TO PICK UP A SELF DUMPING HOPPER. AS HE APPROACHED HOPPER HE TURNED SHARPLY TO LINE UP FORKS. THE TURN WAS TOO SHARP FO R SPEED OF LIFT CAUSING LIFT TIP ONTO RIGHTSIDE BOUNCING ROLL CAGE OFF OF LEFT FOOT. EMPLOYEE WAS NOT PINNED UNDER EQUIPMENT.
