3 P Stone · Caught in, under or between a moving and a stationary object

Standing by roller table at rock chopper while workers were pushing rocks down the line and instead of having hand on top of the rocks EE stuck one in between and when the next rock came down the line it smashed EE's two middle fingers. Breaking both fingers and having to get stitches in one.

Employee was cleaning on the opposite side of the control handles on the rock chopper when the 90 degree elbow broke. Which resulted in them getting burned by hydraulic oil.

EE stuck hand in between chopper blade and rock as chopper blade came down and it smashed it. EE said EE knew better that EE didn't know why EE did it.
