Ee was removing pin on 972G loader. The bucket and link were removed. Ee was pulling pin out of the top z-bar link where it attaches to the tilt cylinder. Ee pulled pin out and the z-bar shifted because pin was slightly over center. The Ee's thumb was caught between the head of the pin and the bucket leveler bracket.
Ee lost footing while installing j-bolt on screen. Ee slipped scraping his right shin and hit his right hip on 2"" channel piece of channel.

EE was assisting in removing pin from a Caterpillar 944 bucket when the pin came out unexpectedly and crushed left hand index finger between pin and bucket.
Ee was pulling on water hose to wash off concrete pad when he stated he felt his wrist pop and noticed pain.

Employees left foot slipped from walkway platform to step causing shin to contact step. Conditions: flat walkway with one step up to another level.
Ee was using sledge hammer on plate to press large bushing into loader (equipment) link. A small piece of metal came off of plate and punctured Ee's left forearm.

Employee was turning over the cutting edge of a 972G CAT loader. As he was loosening the right nut/bolt of the cutting edge the center bolt broke free causing the edge to drop suddenly to the ground on his right foot.
Bolt was being knocked from pin on EC 240 LR Excavator. A punch was struck by another employee to remove pin when the working end of punch broke off and hit employee in the neck.

The injured miner was dismounting a haul truck when he slipped and started to fall. As he was falling he was able to regain his grip with his left hand and the weight of his body caused him to strain his left shoulder.
Employee was removing parking brake assembly from heavy equipment when the parking brake assembly shifted and pinched the right ring finger between the parking brake and the frame of the machine.
The injured miner failed to maintain three points of contact and slipped while climbing the ladder of an intregated tool carrier and the tailbone of his lower back contacted the machine. He reported this accident on the following day and refused medical treatment. Medical attention was requested on 3/14/07.

The affected miner had been welding on guards during his shift and had gone home at the end of his shift. Soon after arriving home, both of his eyes started to hurt and swell closed. He sought immediate medical attention and was diagnosed as having welding flash.
The injured miner was throwing rock from a conveyor at the end of his shift and on his way home felt a strain in the area of his left lower back. This was not reported until the morning of 03/14/06.

The injured employee was checking the shims in the toggle block section of the jaw crusher at the primary side of the plant. He began to climb back down after checking the shims and slipped on a step on the front of the jaw crusher mainframe that had a small spot of oil on it. Due to losing his footing, he fell down to the platform below and injured his back.
The injured miner was attempting to remove a bearing cap bolt while performing maintenance on a jaw crusher with a sledge hammer and another hammer used for driving out pins. As he struck the pin-driving hammer with the sledge hammer trying to drive out the cap bolt, a small piece of the pin-driving hammer broke off an became embedded into the calf muscle on his left leg.
EE was assigned the task to dispense diesel fuel into a bulldozer. EE noticed a rock that was hindering him from getting close enough for the fuel nozzle to reach the dozer. EE attempted to move the rock with the help of coworker. EE strained a muscle on the right side of his lower back.

The employee injured his back moving furniture outside of work back in early March. He was cleared to return to work on March 21, 2004. After two weeks (April 7, 2004) he aggrevated the same area in his lower back while he lifted a piece of metal plate onto a conveyor belt (C-5).

ON ONE LINE, AS HE REMOVED THE PLUG, HYDRAULIC FLUID SHO T OUT * ONTO EE. HE JUMPED OUT FO THE WAY, STRIKING HIS HAND ON THE CRUSHER CAUSING A SPRAIN & BRUISE TO THE LEFT WRIST.
EMPLOYEE LOST HIS FOOTING ON THE STAIRS WHILE WALKING DOWN FROM #2 SCREEN. HE CAUGHT HIMSELF ON HIS WRIST, RESULTING IN A SPRAIN TO HIS RIGHT WRIST.
EMPLOYEE WAS DISMOUNTING THE TRUCK WHILE PARKED AT THE DUMP HOPPER. AS HE STEPPED ONTO THE GROUND, HE FELL BACKWARDS CATCHING HIMSELF WITH HIS LEFT ARM.
ON 6/27, EE REPORTED TO HIS SUPERVISOR THAT HIS BACK WAS SORE AND HE NEEDED A COUPLE TYLENOL. HE SAID THE 40 TON TEREX HE WAS OPERATING RODE ROUGHER THAN THE CAT 777 HE NORMALLY OPERATED. HE F INISHED THE SHIFT AND WORKED 6/28 WITHOUT INCIDENT. HE REPORTED THE INJURY ON FRIDAY 6/30.
