EE reached over the top of engine to take off oil filter and pulled lower back muscle, on Thursday 2/3/05. At 10:30 AM on Monday 2/7/05 he was hammering wedges in on screen plant and pulled same area again. Left work at 12:00 noon to rest strain. After Dr. visit & drug screen was placed on light duty work until re-evaluation on 2/14/05.

ORIG BURN ON OR ABOUT 9-18-03 WHILE USING CUTTING TORCH SLAG ROLLED THRU SLEEVE-WAS UNREPORTEL AND UNATTENDED TO AT THE TIME OF INJURY. WEEK LATER ARM SORE AND SWELLED THEN FEVER. WENT TO DR. FRI 9-26-03 POSSIBLE CELLULITIS-TREATED NO RESTRICTIONS. RETURNED TO DR TUES SEPT 30, DEFINITE CELLULITIS-5 DAYS ANTIBIOTIC TREATMENT. NO WORK UNTIL MON 10-6-03. AS OF 10-6-03 NO RESTRICTIONS
