EE was cutting zip tie off end of ""down the hole"" measuring tape. Cutting away from themself. When the zip tie was cut the knife became uncontrolled and cut another EE. The cut was in EE's hand. The cut required 6 stitches.

Employee was using a utility knife to cut skirtboard rubber and the knife slipped and cut employee's left thigh.

Employee was installing plates on track crusher. Employee was distracted and plate struck the employees hand on the guardrail. Right hand received small deep abrasion.

Employee was trimming grass around the main gate when he hit a bee/wasp nest. Employee was stung several times. Employee was sent to the doctor as a precautionary measure.

Employee was clearing off a screen that was overloaded with material, when a rock fell off and struck his right foot.

Employee was replacing panel on bottom deck of screen. Employee was removing old screen, when employee felt a pull in his lower back.

Contact with hot objects or substances

Employee made contact with a hot piece of metal (area that he previously welded) when reaching for welding rods resulting in a second degree burn on his arm. Burn became infected on the evening of 4/29/09 and employee sought medical treatment.

Struck by falling object

Employee was installing a 4"" valve on a waterline when some loose material slid down the slope and hit the employee's ankle.

Employee was using sledge hammer to remove pins from cylinders on a cone crusher. Employee stated right hand was hurting yesterday evening after he left work. Employee reported the injury on 4/15/09 at beginning of shift.
