Over-exertion (Not Elsewhere Classified)

Supervisor was changing out chemical at wash plant and got it into place. It wasn't square on the stand so they had employees rig chain around top of cage and tried to square it up. The cage broke and two employees tried to stop it and one got a cut on their finger that required 2 stitches and the other got a severe wrist sprain. Neither will miss a scheduled day of work.

Got a finger caught between pulley and belt when attempting to change drive belts.

Accident type, without injuries

Flame detector, detected the igniter sparking & opened up the main gas valve. When the valve opened up, it filled the Plenum with gas. Then when the Pilot light lit, it caused a hard ignition and caused damage to the I-beams that support the floor plates.

Over-exertion in lifting objects

Miner was shoveling loose sand with a standard square shovel when EE strained EE's shoulder.

Accident type, without injuries

Dryer lifted off of the pedestal, caused damage to the internal floor support I-beams. This was caused from too much gas inside of the dryer prior to ignition. Engineer was called to look at the dryer. No major indication of what specifically caused the over gas condition. Troubleshot all gas components, air systems and all systems checked good. Dryer started up per design.

Employee was placing a metal plate under jack of generator that was supported by a chain. A twist in the chain caused it bounce as it straightened pinching employees finger between jack and metal plate. Right middle finger
