Employees were trouble shooting pressure on regulator on vaporizer trailer when contractor opened valve releasing gas causing a flash fire.

IP entered a tank that was off limits to employees. IP entered tank while heated water was flowing into tank to remove built-up sand. IP lost footing and fell into water causing burns to legs and abdomen.
