EE was splicing a belt and the knife EE was using slipped and cut leg above knee. EE received 5 stitches and was drug tested and returned to work that day.

Removing bolt from end of crusher rotor. Struck bolt with hammer, hammer chipped and struck hand. Didn't know there was anything in my hand. Later x-rayed on 5-18-15, showed metal fragment in hand

Struck by falling object

Checking forklift out, side panel fell; hit me in forehead. Went to occ health and they glued the wound shut and gave me a tetnus shot.

Absorption of radiations, caustics, toxic and noxious substances

Hose on 245 excavator broke after lunch startup, so oil was not hot, oil sprayed on operator's face and chest, got in mouth and eyes, flushed eyes at office took to eye doctor, checked out good. Was prescribed an antibotic & inflammation drop, to be put in 1 drop four times a day for seven days. Going to be rechecked Oct 2, returned to work next day 9-29-06.

WHILE INSTALLING A DRIVESHAFT IN A PICKUP TRUCK IT SLIPPED AND STRUCK VICTIM ON HIS FOREHEAD.
