EE was trying to align the drag cable with the drum, so as to be able to wind it properly. EE supported the cable with her right foot. The cable must have caught and everything went right quickly and snapped my ankle. Required surgery ti repair.

While performing maintenance on bucket of the 312 Cat excavator a chip came off one of the teeth when struck with a hammer. The chip struck EE in the face below the left eye beside his nose.

INJURY OCCURRED ON 5-7-01. I WAS INFORMED ON 5-15-01. THIS WAS THE FIRST DAY OF LOST TIME. THE EE RETURNED TO WORK ON 5-17-01. I HAVE ENCLOSED A COPY OF THE INJURY REPORT FROM THE HEALTH CARE FACILITY.
