Received paperwork today that employee is injured. EE is seeing a chiropractor and not a doctor. I am still waiting on additional paper work but I do not believe this is a valid claim. Requested records from primary care doctor. EE told doctor that EE didn't get hurt at work and it was something EE did.

EE started work @ 7am. EE walked on catwalk during morning. Came to office to report broken drag chain. Repair was completed & another repair was decided on fixing tooth on bucket. Bucket needed to be repositioned. EE went to controls. EE walked around on catwalk to engage clutch. After engaging clutch & returning to cab the welds broke on catwalk. EE's arms/armpits caught fall.

Lifting shaker screen that sits on 4 coil springs-as screen lifted only 3 coil springs released but not the 4th. When lifting for 2nd time screen released from 4th coil and slowly swing away from plant and then came back. EE put foot out to stop shaker screen and leg got pinched. EE has denied all drug testing demands/request.
