EE WAS CLEANING ROLLER BEARINGS WITH SOLVENT AND TRIED TO USE COMPRESSED AIR TO BLOW DRY BEARINGS LOOPED OVER SCREW DRIVER. BEARING FLEW APART AND HIT EE ABOVE HIS RIGHT EYE CUTTING SKIN.

CABLE ON CAR AIR LOCK HUNG UP.CABLE EITHER MOVED BY THE EMPLOYEE TRIPPING THE CONTROL VALVE,OR BY HIM FREEING THE CABLE WITH HIS HAND.HIS FINGERS WERE CAUGHT BETWEEN THE 3/16" WIRE ROPE AND TH E PULLEY.
