Employee went to move a hose and punctured finger using a wire hanging off of the hose. Employee did not use appropriate PPE

Employee was using a box cutter to cut conveyor belt in tight area, at the end of the cut, knife slipped and punctured employee's thigh through employee's jeans.
Swinging 20lb sledge hammer in process of removing bolt. Swung and missed the bolt making contact with right lower leg. Right leg contusion.

While changing the discharge location on the sand line, the drop shell fell crushing EE's left index finger between the shelf and the chute.
Putting metal onto saw rollers for cutting, roller fell over pushing metal to saw and crushing right ring finger. Broken right ring finger.
Employee was cleaning debris underneath the jaw crusher as EE was coming out EE hit EE's head and kinked EE's neck. On 4/19/22 EE felt better but was having pain that increased on 4/20/22 which started travelling down leg.

Unhooking heavy rigging from crane, employee lifted incorrectly, and strained lower/mid back.
Employee was adjusting forks on rough terrain forklift and smashed middle finger on right hand between fork and frame. Required 6 stitches.

Adjusting a vent valve on a new air cylinder, the existing airline was connected to the wrong port causing the new cylinder to move in the wrong direction, employee pulled EE's hand back and cut EE's left middle finger on the valve handle. The finger required 4 stitches.

While removing a steel wheel rim from Scrubber Drum assembly rim fell against another metal component, miner's hand was caught between steel rim and the metal component.

Splicing a belt he hit a metal tab on the template and rolled the razor into his left pointer finger requiring 3 stitches.

He was welding at the drum of the scrubber - finishing with a few beads. Raised his hood up - chipped off a couple pieces of slag, one bounced off safety glasses into right eye. Flushed eye several times - next day eye became sore. Piece of slag had embedded and was removed.
Conveyor spilled material onto the catwalk of the scrubber. Employee was shoveling rock off the catwalk and felt a pain in his back. The following day his back was stiff and sore and he sought medical treatment.

Grinding a piece of metal, piece of metal bounced off service truck and entered the side of his safety glasses. Grinder guard in place and safety glasses were worn. Eye flushed and thought particle was washed out. Following morning the eye was sore and red - particle had adhered to the right eye.

Carrying a piece of metal down the embankment, caught his right foot in the water runoff drain and twisted his right knee. Continued working rest of shift. Worked regular shift 10/14/09, knee swelled so went to after hours clinic to be checked out evening of 10/14/09.

Applying pressure with a jack, the jack slipped off the curved surface and struck employee in the face cutting his left eyebrow, nasal and lip contusions.

Cleaning oversized rock out of grizzly bars at the pit hopper. Swinging 10 lb hammer lost footing, slipped and landed on his motorola radio and a piece of steel causing a large hematoma to the right buttock.
Coming down the 3/4-O ramp, the weight in the loader bucket caused the loader to light weight on the rear tires. The bottom of the bucket contacted the ground forcefully jolting the operator and he hit the left side of his head on the cab of the loader.

EE WAS REPLACING THE GRINDING WHEEL ON HIS 4"" GRINDER WITH A 40 GRIT SANDING WHEEL IN ORDER TO ROUGH UP A PIECE OF STEEL TO PUT DOWN NEW RUBBER. THE SANDING WHEEL CAME INTO CONTACT WITH THE OLD PIECE OF RUBBER. GRINDER JUMPED OUT OF C0NTROL & CAME INTO CONTACT WITH THE INDEX & MIDDLE FINGERS OF HIS LEFT HAND.

FELL OFF THE BED OF THE BOOM TRUCK WHEN HE STEPPED AROUND THE CONTROL PANEL FOR THE HOIST. MUDDY GROUND CONDITION & HE DIDN'T CLEAN SOLES OF BOOTS OFF ENOUGH, CAUSING HIM TO SLIP.

EE ACCESSED TANK FROMT HE FRONT, CRAWLING AND STRETCHING OVER BRACES AND SHOULDER HAVE BEEN WORKING FROM THE SIDES POSITION OF HIS BODY CAUSED SEPARATION OF RIB FROM CARTILAGE.
