Employee was removing a fitting from the vacuum truck hose. The fitting was stuck and as the employee tried to spin it to free it up his thumb slid across a lip inside the fitting, cutting through his glove into his thumb. The employee was taken to urgent care for medical evaluation and received 5 stitches

Employee was setting up job at primary crusher apron feeder. While throwing a welding lead down into the apron feeder, the employee felt a sharp pain in his shoulder and lower left arm.
Employee was moving the godwin water pump at quarry #3. As he was walking up the gravel road, he slipped on the wet gravel falling to the ground. He suffered a cut on his LT wrist which required some stitches.

EMPLOYEE WAS APPLYING GREASE TO THE GREASE FITTINGS ON A MANLIFT ELEVATOR SHAFT BEARINGS HE THOUGHT THAT ONE OF THE FITTINS WASNOT GETTING ANY GREASE SO HE STEPPED DOWN INTO THE PIT 36IN DEEP TO CLEAN AROUND FITTING THIS FITTING LOCATED BETWEEN THE WALL AND MANLIFT HE LOST HIS BALANCE AND REACHED FOR FRAME FOR SUPPORT HE ACCIDENTLY PULLED ON CABLE WHICH ACTIVATED START UP SWITCH ST
THE EMPLOYEE WAS OPERATING A TRACTOR TRAILER ON COMPANY PROPERTY AT PLANT #1, NAZARETH. AS HE WAS ENTERING THE CLINKER STORAGTE FIELD, A FRONT SHEEL WENT INTO A POT HOLE. THIS CAUSED THE STEE RING WHEEL TO SPIN AROUND. HIS LEFT HAND WAS STRUCK BY ONE OF THE INNER SPOKES OF THE STEERING WHEEL.
A LARGE PIECE OF CEMENT ROCK LODGED ITSELF INSIDE THE STONE CRUSHER CAUSING A CHOKE THE EMPLOYEE ALONG WITH 2 OTHER MILLWRIGHTS HAD PLACED A STEEL CABLE AROUND THE STONE SO THAT IT COULD BE M ANIPULATED INTO A POSITION TO BE BROKEN UP THE EMPLOYEE WITH THE UPPER PART OF HIS BODY REACHED INTO THE INSPECTION DOOR OF THE CRUSHER THE ROCK MOVED AND PINNED HIS BODY IN THE INSPECTION
THE EMPLOYEES RIGHT LEG WENT THROUGH THE STEEL GRIZZLEY UP TO HIS KNEE WHEN HE AND ANOTHER MILLWRIGHT, WERE MAKING A RESUE ATTEMPT TO FREE ANOTHER EMPLOYEE WHO HAD GOTTEN HIMSELF PINNED IN THE INSPECTION DOOR.
