While checking equipment in back portion of dragline, employee's foot caught on belt guard causing employee to trip and fall fracturing left knee cap.

While moving lubrication oil barrel, barrel slipped from one employee's hand and pinched other employee's finger.

When attempting to move a skid steer bucket, on a table, for repairs, employees left hand got caught between the bucket and table, injuring his hand.

Fall onto or against objects

As employee approached a ladder to descend from a dragline, he lost his footing and fell onto the dragline ""shoe""; he fell on the same level at which he was walking. He was given treatment by EMTs and taken to the emergency room. Employee has sore back muscles and ribs.

Pulling table bushing out of a drill and aggravated his hernia from July, 2011. Went to see a doctor on September 27, 2011.

Over-exertion in pulling or pushing objects

Pulling on heavy dragline chain to get it in place. Employee did not seek medical advice until approximately a week later and then was diagnosed with a hernia. After a follow up consultation with another doctor on 7/1/2011, employee in the near future is looking to have hernia surgery.

While carrying lunch pail in one hand, was holding on to climb down rail on front end loader with other hand. Missed bottom step with foot, was still holding onto handrail and stretched. Employer was informed by employee on 5/7/2010 that this incident caused a tear in rotator cuff.

While one EE went to get the planks to place over open floor area, inj EE (before planks were placed over open area) removed chain from protected area, cont'd to remove guard over conveyor, stepped down to where the planks would had been placed and fell 8-10' cracking 3 vertebrae.

Employee pulled too hard, on a 7' long 4x4 piece of hardwood that was being used to create a lever to close gate on crusher, receiving sharp pain in groin area.

VICE GRIP PLIERS WAS HOLDING PIECE OF FLAT PLATE STEEL TEMPORARILY. EMPLOYEE WAS LEANING ON THE PIECE OF STEEL WHEN HE REMOVED THE VICE GRIP PLIERS, CAUSING EE TO LOSE HIS BALANCE & FALL.
