Employee had just finished cutting a bent handrail with cutting torches. EE hit EE's left forearm on the handrail and received a laceration. The laceration required three sutures.
Employee was removing snow from screen deck and they slipped and fell into chute. Employee suffered a right MCL sprain and a right tibia fracture.

Employee was helping to install the shaft in a log washer. EE was pushing the shaft into place with legs when it shifted and jammed EE's knee. EE was diagnosed with a right knee strain.
2013 · 2 incidents
Employee exited the CAT D6 Dozer because the fire extinguisher was rattling against the window. When he went to re-enter the cab, he took a step and his foot missed the track. He fell to the ground injuring his back and left hip.
Employee was walking down cat walk near main screen. His foot slipped when he stepped on a piece of rubber and he felt a pop in his left thigh. He had an MRI which showed a quad tear. He will have surgery to repair the injury.
