Struck against a moving object

Employee was operating a 50 ton Cat end dump truck & was at dump site with his 3rd load of stone from pit. While attempting to dump the load from truck at dump site, which is elevated some 20' above the plant, his truck went over the dump face backwards & flipped end for end 180' landing on its top, wheels to sky, at bottom of dump site at plant level. He was still in cab.

Fall from ladders

Mechanic was disassembling tin shed and climbed ladder to cut a bolt holding shed wall. Fell backwards off of ladder and hurt his ribs.

WHILE OPERATING BLADE, EE DIRECTED HIS ATTENTIONAWAY FROM THE DIRECTION OF TRAVEL. DURING THIS BRIEF PERIOD OF TIME, THE EQUIPMENT STRUCK AN EMBANKMENT, JARRING HIM UNEXPECTEDLY, RESULTING IN A PULLED BACK MUSCLE.
