While attempting to dump the load from truck at dump site, which is elevated some 20' above the plant, his truck went over the dump face backwards & flipped end for end 180' landing on its top, wheels to sky, at bottom of dump site at plant level. He was still in cab.

THERE WAS A LARGE ROCK STUCK IN THE PRIMARY JAW CRUSHER. EE TRIED TO MOVE THE ROCK WITH A BAR. THE BAR SLIPPED AND HIS LEFTTHUMB WAS SMASHED BETWEENTHE BAR AND THE HAND RAILING.

EMPLOYEE WAS TIGHTENING BOLTS ON CONVEYOR FRAME.BEING REPAIRED. WRENCH SLIPPED OFF NUT ALLOWING HIS HAND TO STRIKE HIS HAND TO STRIKE AGAINST METAL FRAME WORK CUTTING HIS HAND.
EMPLOYEE WAS REPAIRING ROCK TRUCK. STARTED TO CLIMB DOWN FOR MORE TOOLS. HAND WERE FREE, BIFOCALS CAUSED MISJUDGEMENT ON FOOTING. MISSTED STEP AND FELL ABOUT 3' STRIKING LEFT SIDE OF HEAD NE AR LEFT EAR.
EMPLOYEE WAS REMOVING OVERBURDEN WITH A SCRAPER, IN A PUSH-PULL COMBINATION WITH A DOZER. THE SCRAPER SEPARATED FROM THE DOZER THEN SLOWED AT WHICH TIME THE DOZER CAUGHT UP TO CONTINUE PUSHING AND BUMPED. THE BUMP JARRED THE SCRAPER OPERATOR'S BACK.
EMPL HELPING ASSEMBLE PORTABLE CRUSHER.WAS WORKING ON SIDE OF MACHINE LINING UP BOLTS HOLES AND TRIED TO ADJUST POSITION WAS STANDING.WHEN HE MOVED HE SLIPPED AND FELL 8' TO GROUND INJURING WR IST AND FACE RESULTING IN BROKEN WRIST & CONCUSSION.
