Struck against stationary object

Employee was climbing down from pay loader when they slipped and lost their step causing their right hand to hit hard on rail above. Employee worked 5 days thinking they had only bruised the hand. On the 31st they went to doctor and found that they had fractured the 5th metacarpal in the right hand.

Contact with hot objects or substances

Individual was clearing blockage from the exit of the hydrator near the exhaust stack when hot hydrate material let loose and sprayed across both his arms. Hot material caused burns to both his arms. It was not determined until 11/30/15 that his injury was such that he could not return to work.

While bagging Hydrate lime a 50# bag split at top forcing lime into employee's mouth. Individual cleared debris from mouth and continued working. Two days later individual had ulcers in mouth and went to doctor for treatment.

Employee was changing propane bottle on forklift when bottle slipped out of his hand causing hand to strike roll bar. Individual fractured 5th metacarpal. Individual did not report incident for 3 days.

While performing routine job on M-4 stone conveyor employee bumped safety switch shutting down conveyor. Employee quickly turned to re-set switch and allegedly twisted his left knee. Employee worked without restrictions until 6/19/14 when he went in for Orthoscopic surgery.

Employee was removing storage silo lid to measure contents and got lime in his left eye. Process was running to that silo making it positive in pressure forcing dust out the opening and under bottom of his safety glasses.

While removing an empty oil drum off storage rack employee allegedly sprained lower back and hip.

Employee was pulling on bar trying to move switch. Bar slipped and employee fell backwards hitting his head on wall of shanty. When he fell he jerked his head and his hardhat fell off.

While carrying 3 rotary kiln fire brick, employee tripped and fell smashing his finger between the brick and the kiln.

Employee was cleaning a scrubber, he had his arm around a water pipe and the ladder he was standing on slipped, causing strain to the left shoulder

Employee was finished loading a truck and started down the ladder and his left hand lost its grip, causing his left knee to strike a ring on the ladder.

Employee was pulling klinkers when the puller stopped, it hit the steel plate holding up the door. This caused a jolt and strain the the body.

Employee was seperating east water in order to hook up bypass. He used a hammer to get coupling apart, when he swung the hammer the coupling came apart and came down about 2 1/2 ft, hitting him on the middle left finger.

When using an industrial kiln gun to shoot ash ring in the kiln a shell exploded injuring the right eye. As a result of the injury the employee lost his right eye.
