Employee was climbing down from pay loader when they slipped and lost their step causing their right hand to hit hard on rail above. Employee worked 5 days thinking they had only bruised the hand. On the 31st they went to doctor and found that they had fractured the 5th metacarpal in the right hand.
Individual was clearing blockage from the exit of the hydrator near the exhaust stack when hot hydrate material let loose and sprayed across both his arms. Hot material caused burns to both his arms. It was not determined until 11/30/15 that his injury was such that he could not return to work.
While bagging Hydrate lime a 50# bag split at top forcing lime into employee's mouth. Individual cleared debris from mouth and continued working. Two days later individual had ulcers in mouth and went to doctor for treatment.
Employee was changing propane bottle on forklift when bottle slipped out of his hand causing hand to strike roll bar. Individual fractured 5th metacarpal. Individual did not report incident for 3 days.
While performing routine job on M-4 stone conveyor employee bumped safety switch shutting down conveyor. Employee quickly turned to re-set switch and allegedly twisted his left knee. Employee worked without restrictions until 6/19/14 when he went in for Orthoscopic surgery.
Employee was removing storage silo lid to measure contents and got lime in his left eye. Process was running to that silo making it positive in pressure forcing dust out the opening and under bottom of his safety glasses.
While removing an empty oil drum off storage rack employee allegedly sprained lower back and hip.
Employee was pulling on bar trying to move switch. Bar slipped and employee fell backwards hitting his head on wall of shanty. When he fell he jerked his head and his hardhat fell off.
While carrying 3 rotary kiln fire brick, employee tripped and fell smashing his finger between the brick and the kiln.
Employee went up stairs to take a chain off door to close, lime fell from the door and received lime on face and in both eyes
Employee was working on coal mill blower and slipped going after tools and twisted his knee. Mercer Lime was notified on April 1 that he would be going to physical therapy.
Employee was turning and twisting wheel at bottom of #1 Chemical bin and felt a pull when exerting turning the wheel.
Employee was trying to shutdown the panel board for the premix screw because lime was blowing out of the screw. Lime blew into his right eye.
Employee was lifting a motor to replace it, when he felt pain in his lower pack and hips.
Employee was handing shells to employee shooting kiln and lime from his hard hat accumulated and blew into his eye.
Employee was walking through plant and slipped on ice and snow and fell.
Employee was pushing the ash ring when he pulled muscles in his lower back.
Employee was taking pop cans to container, stepped on cover of man hole. The cover came off and employee fell into man hole.
Employee was bagging lime and he tapped the bag and the bag ripped and lime blew in his eye, causing lime burns.
Employee was pulling klinkers and experienced pain in lower back and down both legs
Employee was cleaning volume box on hydrator. His right leg slipped and left knee buckled, causing pain and swelling.
Employee was lifting a drum and the drum slipped catching his finger between the drum and a fire extinguisher box
Employee was cleaning a scrubber, he had his arm around a water pipe and the ladder he was standing on slipped, causing strain to the left shoulder
Employee was finished loading a truck and started down the ladder and his left hand lost its grip, causing his left knee to strike a ring on the ladder.
Employee was pulling klinkers when the puller stopped, it hit the steel plate holding up the door. This caused a jolt and strain the the body.
Employee was seperating east water in order to hook up bypass. He used a hammer to get coupling apart, when he swung the hammer the coupling came apart and came down about 2 1/2 ft, hitting him on the middle left finger.
When using an industrial kiln gun to shoot ash ring in the kiln a shell exploded injuring the right eye. As a result of the injury the employee lost his right eye.
Pulling klinkers, he felt severe pain in shoulder and neck.
Pulling klinkers from the kiln, he strained back.
Employee was running the chemical bagger and got his hand caught between two full bags of lime as they were dropping onto the belt.
Employee was pulling klinkers and pulled his left arm muscles. This was not reported earlier because he did not seek medical attn or miss any work until March 18, 2008
Employee was walking down a ramp behind coal building and slipped and fell on snow and ice, landing on his left hip. This accident was not reportable until 10/17/08.
Employee tripped over a grinder that was laying on the floor.
Cleaning scrubber and hit left elbow on side of scrubber. Employee worked from date of accident until 12/2/07. Last day worked 12/2/07.
Pulling klinkers he pulled a muscle between shoulders and neck.
Pulling clinkers and felt pulling in his lower back. Sprain/strain to the lower back.
Employee was walking to payloader and slipped on ice and fell onto his back.
Filling Bags and moving pallets onto scales. Felt pain in his neck and back

Helping to pull merrick stone belt through roller. Note: this accident was not reported earlier because we did not know that surgery would be involved, or any lost time.
Employee was putting a piece of steel onto a stack of steel, and it rolled back off stack and onto his foot.
EMPLOYEE WAS CLEANING OUT A FAN HOUSING AND SPRAINED HIS WRIST.
EMPLOYEE WAS CLEANING THE STONE ROOM & WAS MOVING A WHEELBARROW TO EMPTY IT & STEPPED ON A STONE & TWISTED HIS KNEE.
PULLING KLINDERS FROM THE KILN AND STRAINED LOWER BACK. REPORT NOT FILED EARLIER BECAUSE EE WAS NOT OFF WORK UNTIL NOW.
EMPLOYEE WAS IN A CAGE AND THE CAGE FELL, COMING TO A SUDDEN STOP AND HE JAMMED BOTH WRISTS AND HIT ELBOW.
EE WAS REMOVING ASH KLINKERS FROM KILN GRATE USING PULLER PROVIDED WHEN HE PULLED OR STRAINED HIS MUSCLE IN MIDDLE OF BACK.
EE WAS SHOVELING LIME FROM A ROOF & PULLED OR STRAINED BACK MUSCLE.
E WAS CLEANING BAGHOUSE PIPE ON SECOND FLOOR, STEPPED BACKWARDS OFF PLATFORM & FELL DOWN THE STEPS.
EMPLOYEE WAS PULLING NEW SHAFT & SPROCKET UP TO TOP OF ELEVATOR AND PULLED MUSCLE IN RIGHT SHOULDER.
EE WAS CLEANING UP LIME IN AN ELEVATOR PIT. HE STRAINED HIS BACK.
EE WAS CLEANING OUT DEMISTER CONE WITH A SLEDGE HAMEER. CONE FELL TOWARDS EE WHILE HE WAS SWINGING HAMMER, PINCHING FINGER BETWEEN CONE AND SLEDGE HAMMER.
EE WAS TAKING NUTS OFF A BOLT AND LOST GRIP WITHPIPE WRENCH, HE WAS USING A PIPE FOR EXTENTION FOR LEVERAGE AND WHEN HE LOST GRIP ON BOLT, PIPESTRUCK HIM ON THE HEAD.
EE WAS POKING OUT ANALYZER PIPE AND A PAIN WENT UP RIGHT SIDE OF HIS NECK.
EMPLOYEE WAS PULLING A LEVER TO RELEASE RAILROD CAR BRAKE AND PULLED A MUSCLE IN HIS SHOULDER.
EE WAS WORKING OFF A WOODEN LADDER, WHEN THE 4TH RUNG BROKE. HE FELL THROUGH TO TOP OF PRE MIX XCREEN.
HIT HIS HEAD ON A STEEL PIPE WHILE GOING UP TO GET BIN MEASUREMENT.
BAGGING LIME AND LIME BLEW INTO HIS EYE.
PUTTING A ROOF BACK OVER WEST FAN AND PULLED HIS LOWER BACK.
BAGGING LIME ADD BAG BLEW UP AND RECEIVED LIME IN HIS EYE.
