Miner was using a prybar. Prybar slipped, striking miner in face/right eye, causing them to fall on their back. Fractures to right orbital socket, 2 broken ribs.

Worker picked up a sheet of plywood, and heard a popping in elbow, resulting in immediate pain, and could not use the arm or hand afterward.

EE then started to push the truck & his hand slipped off the door jam, hit the grab handle, breaking it from the force from slipping & then cut his hand on the broken piece that was still attached to the truck.

WORKER WAS PROCEEDING TO GET DOWN OFF OF BULLDOZER. HE SLIPPED OFF TRACK, FELL AND HIT BLADE SIDE BAR, TRACK AND GROUND BRUISING ELBOW AND BRUISING AND TWISTING ANKLE.
