Miner was using a prybar. Prybar slipped, striking miner in face/right eye, causing them to fall on their back. Fractures to right orbital socket, 2 broken ribs.

Worker picked up a sheet of plywood, and heard a popping in elbow, resulting in immediate pain, and could not use the arm or hand afterward.

Miner was assisting welding operations, and complained of symptoms consistent with arc flash, the day following that task. Miner did not report this at end of shift, but sent a text early the next morning.

A pickup truck wouldn't start. It was in the way for dump truck to back up. Owner of Alliance Sand told Employee to push it out of the way. EE then started to push the truck & his hand slipped off the door jam, hit the grab handle, breaking it from the force from slipping & then cut his hand on the broken piece that was still attached to the truck.
