IW was using a cutting torch. A piece of metal came loose and struck IW right hand causing a laceration. The metal also struck IW right foot causing a contusion.

Flash on the handle of welding torch. Flame went under injured worker's glove.

Walking on the ground.

EE was removing bolts from bearing flange and bolt hit EE in the head.

A piece of plate steel hit and bent thumbs backward.

EE fell off uke bumper while entering the truck. EE injured right wrist, elbow and ribs.

Using hand wrench to loosen a nut. Wrench slipped off and elbow hit pipe below.

Employee was adjusting the gates on the screen deck of the crusher and he knelt on a rock. The knee became swollen.

Employee was installing a steering wheel lock on a loader and lifted the ram of a 20 ton porta power and tore something in his left elbow.

Employee was cutting steel with a torch and a piece of hot slag flew into his right eye.

Employee was kneeling for an extended period of time doing maintenance on a jaw of a crusher changing a jaw die. Initially there was no medical treatment required. On 5/31/12 the employee asked to seek medical treatment for a sore/swollen right knee. He was scheduled for an appointment for 6/1/12. He is now working under restricted work duty.

Employee was cutting a piece of conveyor belt with a utility knife and cut his left thumb, requiring stitches.

Employee was walking on the crusher deck under the wet screens and hit his head on a pipe, wrenching his neck. He was wearing his hard hat.

Employee was taking garbage to a dumpster and slipped on ice. Bruised his right shoulder.

Employee was pulling a dragline bucket chain and strained his upper back.

EE was working on a scalper deck of a crushing plant. He got his finger between a bar and hammer and lacerated his right middle finger. EE did not report this injury until 10/13/09 when it became infected.

Employee was changing a cable on a dragline and a strand of cable punctured his right forearm. He was sent to the emergency room for a tetanus shot.

Employee was removing a rock from between the tires of a water truck. The tire blew out and caused minor scratches to his face and debris into his eyes.

Employee was using a cutting torch to remove the old steel floor liner in a type B sand screw. Previously someone made a repair with crusher backing. When the employee finished cutting he removed his goggles, unaware that there was crusher backing there. Since the crusher backing had been heated a piece popped off and struck him in his left eye.

Employee was greasing a dragline; when he stood up he heard a popping sound in his back.

Employee was dumping asphalt millings. Material would not come out of the truck body, truck layed over on its right side.

Employee slipped from a ladder on a loader and sprained his ankle.

Employee twisted his right knee while scraping a belt.

Employee caught foot between two pieces of steel. Twisted right knee.

Employee was cleaning a wall air conditioner and cut his left index finger.

EMPLOYEE WAS CLIMBING DOWN THE STEPS OF A LOADER AND SLIPPED OFF THE STEP HITTING HIS RIGHT KNEE ON THE GARAGE FLOOR.

GETTING IN CAR EMPLOYEE SLIPPED ON ICE TWISTED LOWER BACK.
