Using hand wrench to loosen a nut. Wrench slipped off and elbow hit pipe below.

Employee was installing a steering wheel lock on a loader and lifted the ram of a 20 ton porta power and tore something in his left elbow.
Employee was cutting steel with a torch and a piece of hot slag flew into his right eye.
Employee was kneeling for an extended period of time doing maintenance on a jaw of a crusher changing a jaw die. Initially there was no medical treatment required. On 5/31/12 the employee asked to seek medical treatment for a sore/swollen right knee. He was scheduled for an appointment for 6/1/12. He is now working under restricted work duty.

Employee was walking on the crusher deck under the wet screens and hit his head on a pipe, wrenching his neck. He was wearing his hard hat.
Employee was taking garbage to a dumpster and slipped on ice. Bruised his right shoulder.

EE was working on a scalper deck of a crushing plant. He got his finger between a bar and hammer and lacerated his right middle finger. EE did not report this injury until 10/13/09 when it became infected.

Employee was changing a cable on a dragline and a strand of cable punctured his right forearm. He was sent to the emergency room for a tetanus shot.
Employee was removing a rock from between the tires of a water truck. The tire blew out and caused minor scratches to his face and debris into his eyes.
Employee was using a cutting torch to remove the old steel floor liner in a type B sand screw. Previously someone made a repair with crusher backing. When the employee finished cutting he removed his goggles, unaware that there was crusher backing there. Since the crusher backing had been heated a piece popped off and struck him in his left eye.
