Employee's knee popped while walking up incline at plant. Management saw employee limping a week later and questioned if he was hurt on the job. Employee said no. On 12/8/10 management received call from doctor saying employee was there saying he had work related injury. Employee was put on restricted duty until release of 12/30/10. No tear in knee.

Fall from machine

EMPLOYEE WAS OPERATING VEHICLE DURING RAINY CONDITIONS. VEHICLE WAS A LOADER. EMPLOYEE WAS COMING OUT OF VEHICLE ONTO LADDER WHEN HE LOST HIS FOOTING AND SLIPPED OFF THE LADDER. HE FELL TO THE GROUND FROM 8' HIGH DROP.

Struck by falling object

EMPLOYEE WAS REMOVING ON ANGLE FROM PILE WHEN HE ACCIDENTLY KICKED PILE TRYING TO ADJUST WEIGHT TO LIFT AND THE ANGLE UNDERNEATH THE ONE HE LIFTED FELL ONTO EMPLOYEE'S FOOT.
