Employee was helping a truck driver get a rock out of the tailgate of their dump truck. As employee and truck driver pulled the tailgate open, the rock fell out, truck driver let go of tailgate, the tailgate swung shut and struck the employee's right pointer finger. Employee's right pointer finger was fractured.

Changing bearing on conv. belt. While taking old bearing off (515/16) bearing slipped & dropped onto EE's finger pinching it between bearing & frame work. Bearing as approx 15 lb.

AFTER EMPLOYEE REMOVED A PIECE OF WOOD FROM UNDER THE CONVEYOR BELT HE TURNED TO GO TO THE CATWALK TO RESET THE TRIP CABLE. AFTER WALKING A FEW STEPS HE FELT A SHARP PAIN IN HIS CALF OF HIS LEFT LEG.

Ee was removing a frozen valve 3" to thaw it out once all bolts were removed the valve was stuck in place. He pulled on valve when it broke loose it. He did not have a good grip on it and it fell on his wrist the valve weighed approx. 40 lb.
EE WAS USING A PRYBAR TO MOVE A PIECE OF HEAVY STEEL WHEN THE PRYBAR SLIPPED PINCHING HIS FINGER BETWEEN THE PRYBAR AND A NEARBY I BEAM. ADEQUATE INFORMATION WAS NOT PROVIDED TO DETERMINE REPORTABLE TO THE MAIN OFFICE UNTIL 1-12-06.

Employee was assisting co-worker to weld pipe together. He was not wearing safety glasses or safety shield to cover his eyes so he suffered welder's flash. He did not see a doctor but missed one day of work and then returned.
Ee was assisting mechanic & dragline operator in changing dragline cable. Ee claims pain in lower back, reported to foreman on 7/28. Stated it will be ok in a few days, reported to work 7/29 & 7/30. About 11:00 a.m. on 7/30 ee advised he wanted to leave early to see doctor.
