Claimant was walking from the scale house to the screen plant and slipped on ice and fell. When claimant fell, his ankle buckled.

Employee was attempting to climb the ladder to the loader, employee slipped and grabbed the grab bar with left arm to avoid falling, employee pulled something in left arm/shoulder.

Individual was moving an oxygen tank when left hand came into contact with sharp metal leaning against a wall. Individual received a 1-1/2 inch laceration between ring finger and pinky finger on back of left hand. Individual received 6 stitches.

INJURED WALKED AROUND BACK OF LOADER, WENT TO STEP UP AND HIS FOOT SLIPPED, CAUSING INJURED TO FALL BACKWARDS OFF THE MACHINE, HITTING HIS HEAD AND BACK ON THE GROUND.

Injured was lifting bags of lime into lime box when he felt a sharp pain in his back.

INJURED WAS LIFTING A BATTERY CHARGER ONTO THE BACK OF A PICKUP TRUCK.

INJURED WAS STRUCK BY A WRENCH.

Injured was lifting sample bags onto truck bed and turned body to the left when he felt his knee pop and start to burn.

Injured was turning handle on a winch when the handle came out of his hand and reversed direction, striking him on the right arm.

DURING THE OPERATION OF THE FRONT-END LOADER A FIRE STARTED BENEATH THE CAB. INJURED BURNED THE PALMS OF HIS HANDS AS HE EXITED THE CAB.

EE WAS STEPPING UP ONTO THE SCALE PLATFORM WHEN SHE TRIPPED AND FELL STRIKING HER LEFT KNEE ON THE TOP OF THE PLATFORM CAUSING A 1/2"" HORIZONTALLACERATION ON HER KNEE.

INJ WAS EXITING THE FRONT END LOADER WHEN THE DOOR SLAMMED SHUT ON HIS LEFT HAND
