EMPLOYEE WAS REFUELING HIS LOADER AND WAS IN THE PROCESS OF POSITIONING THE STEEL SAFETY LOCK WHEN HE SLIPPED AND THE SAFETY LOCK FELL ON HIS THUMB FRACTURING IT.

55061S INA MANHOLE CLEANING A FILTER ON A WATER PUMP AND WAS BEING BITTEN BY ANTS. EVEN THOUGH HE WAS ADVISED NOT TO HE PROCEEDED TO POUR GASOLINE AROUND THE TOP OF THE MANHOLE. INORDER TO KIL 30385 ANTS THEN HE CLIMBED BACK INTO THE MANHOLEAND TOLD A CO WORKER TO TURN ON THE PUM THE STARTING OF THE PUMP TRIGGERED AN EXPLOSION CAUSING THE INJURIES.
