EE was using a sledge hammer to tighten down nut on crusher mantle liner. He struck his right thumb nailbed with the hammer.
Employee was tightening up a come-a-long and the chain broke. Chain hit employees rt hand, and broke rt middle finger.
Employee was asked to use a shovel to clear dirt covering a pipe. When he lifted the shovel at one point he felt a tightening and pain in his back.

EE NOTIFIED ME JUNE 13,1993 THAT HIS NECK HURT WEHN HE TURNED TO LOOK CENTEN DIRECTIONS THAT WAS GOING TO HAVE IT CHECKED BY HIS DR AND WOULD LET ME KNOW WHAT WAS HAPPENING DR PUT HIM ON MEDIC ATION AND HE WOULD NOT COME TO WORK DUE TO MEDICATION HE WAS TAKING.
EMPLOYEE WAS CUTTING RUBBER CONVEYOR BELTING WITH RAZOR KNIFE, KNIFE SLIPPED OFF MATERIAL AND PUNCTURED EMPLOYEE'S LEG (RT) AT KNEE. NO STICHES WERE REQUIRED, BUT LEG BECAME SWOLLEN AND HE WEN T TO DR. AND WAS TOLD TO STAY HOME.
EMPLOYEE WAS WORKING WITH 24" PIPE WRENCH DIS CONNECTING TWO PIPES WHEN HE DROPPED WRENCH ON RIGHT HAND INDEX FINGER WAS NOT CUT OR OPEN JUSTSOME SWELLING AND DISCOMFORT UNTIL 4-22-93 HE HAD DISCOMFORT AND WANTED TO GO TO MEDICAL CTR TO HAVE CHECKED. RETURNED TO WORK UNTIL 4-26-93 HE TOLD TO CUT HIS FINGER OPEN AND HE WOULD BE OFF 2 WKS.
