Employee was using a razor Blade knife to cut a nylon rope. The blade cut though the nylon rope and contacted the employees left forearm causing a laceration.

Miner was standing between roof bolts when a piece of rock (grey shale) fell from between the bolts and contacted the injured miner. The area where the accident occurred was heavily rock dusted which made sight of fracture lines hard to see.

A miner was injured while in the process of changing the canopy post on a twin boom Roof Bolter in MMU 006-0. As the canopy was being lifted off the operators' side, the canopy fell and caused pinched tip of the injured miners' finger.

A miner was injured when ee placed hand on the chuck of the roof bolter head to check for suction. EE right ring ringer was injured when a drill steel fell and contacted the tip of finger.

Dirt/debris entered the injured miners left eye on 05/26/2017. Medical treatment was requested on 06/05/2017 because of a continuing eye irritation. The miner was prescribed eye drops on 06/06/2017.

A miner was injured while striking a chisel with a hammer. A piece of rust/metal went under the injured miners safety glasses frame and contacted the left eye. Employee was wearing prescription safety glasses when the incident occurred. Prescription eye drops were prescribed because of the scratch on the left eye.

A miner was injured when employee pinched the tip of employee's left little finger while working on the Fletcher roof bolter.

A group of miners was changing the conveyor belt on number 4 belt line. A miner was injured when EE was struck on the left forearm by a belt grab. The belt grab broke left ulna bone.

A miner was injured starting to drill a hole in the roof where adverse conditions existed. A piece of rock measuring 10" X 16" by 4" thick fell from the roof and contacted the metacarpal glove on right hand. The rock was sharp and it cut through the glove. Employee received a laceration on two fingers on right hand.

On December 21, 2016, a miner complained of a painful swollen right knee. The injured miner stated they didn't do anything to injure the knee. The injured miner said the swelling must be from crawling through the section. Employee was seen by the Emergency Room doctor. The doctor diagnosed the swollen knee as bursitis and drained fluid from the knee.

A miner was injured while changing a scoop universal joint. EE struck the universal joint cap and a sliver of metal became dislodged and struck EE in the abdomen. EE was transported to Conemaugh Emergency room to be treated. The doctor lanced EE's abdomen to remove the metal sliver and because of the lace, EE received three sutures.

A miner was injured while loading 6 inch plastic water line on a scoop. When the 6 inch plastic waterline was placed on the scoop, the end closest to the injured miner bounced up and contacted the miner's safety glasses. The safety glasses caused a small laceration under left eyebrow. The hospital placed three sutures in the affected area to close the laceration.

The miner was injured while walking along the belt-line. EE stepped into slippery mud, lost EE's footing and fell. EE's right ear contacted the metal structure and caused a laceration on EE's right ear.

Changing inserts on continuous miner head. Metal on metal contact caused a metal sliver to become dislodged and penetrated the injured miner's wrist. Doctor removed the metal from EE's wrist on 4/14/2016 and EE received several stitches to close the wound.

A roof fall was discovered at 4:50 PM. The fall is located in the #6 entry 50 feet inby survey station 184. The fall is approximately 65 feet long by 17 feet wide. It is tapered on all four sides and ranges in thickness from 0 to 8 feet. The area was originally bolted in 2001 with 6 foot fully grouted resin bolts.

An unintentional roof fall occurred on the number 1 belt tail. The fall occurred in the number 4 entry at survey station number 63. The size of the fall is 12 foot 4 inches wide by 12 foot one inch long by approximately five foot high.

The injured miner pinched his finger between messenger cable and a conveyer roller as he was installing a new belt.

A miner sustained an injury to his right foot while preparing to install a roof bolt. A piece of laminated rock fell from the roof, contacted the drill station canopy, and came to rest on the injured miner's right foot. The work area was approximately 80 inches high.

An unitentional roof fall occurred on the F mains section from number 3 entry to number 4 entry fall was approx. 80 foot long by 20 foot wide height is unknown.

Employee was trouble shooting a valve body on a joy miner. When he lowered the heads of the machine they contacted the bottom causing the rear of the machine to lower onto his foot causing three (3) broken toes and lacerations to two (2) of his toes

Employee was standing on a tongue of a supply trailer getting ready to hook up hydraulic lines when he jumped off of the tongue and landed on a rock spraining his right ankle

Employee was in number 4 entry operating the miner when a section of rib and roof rock fell striking him on the back the rock measured 43" long 8" wide and 12" thick employee received brush burns and a bruised back

Employee was going to remove a fire hose from a fire valve when he went to the fire valve and knelt down the hose blew off of the valve striking the employee in the right eye causing a laceration to his upper eye lid and right cheek he also received a broken bone under his right eye

Employee was shoveling the 5-6 entry along the rib line when a section of top rock and coal fell striking the employee on the right thumb causing a laceration and a broken thumb.

A miner was installing roof bolts with straps in an area with adverse conditions. The miner was injured when a piece of sharp soap stone fell from the roof and contacted his right foot. The rock fell from between the last row of bolts and the coal rib.

On October 22, 2014, the named employee exercised his rights afforded under the Federal Regulations Part 90 to work in a low dust area at the mine.

A miner was injured while installing roof bolts in adverse conditions in the #5 entry of the 7 mains section. A piece of roof rock came loose and fell on the bolter head. The piece of rock ricocheted off the head and hit the injured miner on the right side of his face causing a laceration.

Employee was lifting belt structure and felt a small amount of pain in his lower back.

Employee was walking with his head down to the roof bolter when a small piece of draw rock fell from the roof and struck him in the lower back.

The man was swinging a hammer to install a bit insert on the continuous miner. While swinging the hammer, the handle broke and the man's hand struck a miner bit.

The man had repaired the shuttle car cable. He was sitting in the shuttle car winding up the cable slack. He leaned out of car to see how much slack was left when the cable came up quickly and struck him on the head.

Two men were knocking down a block wall. While one person was picking up block, the other man hit a row of block hanging from the roof causing the blocks to fall and strike his co-worker.

The man was installing a bottom belt hanger when the belt chain to the come-along broke and pinched the man's finger. Report filed for Medical Treatment Only(sutures)

The man was using a ladder to remove a mine phone from a storage shelf. While stepping down off the ladder, the man slipped and fell onto the floor.

The man was crawling in the section when he reported low back pain.

On January 27, 2014, the named employee exercised his rights afforded under the Federal Regulations Part 90 to work in a low dust area at the mine.

The employee was pulling a miner cable along with another employee. While attempting to throw the cable along the rib, the other employee gave the cable a strong pull causing it to come up and struck the person in the face and knocking off his safety glasses.

The man was bending a roof bolt when he felt pain in his hand.

The man was installing roof bolts in a crosscut. As he started to drill a bolt hole, a piece of rock fell and struck the man on the foot.

The man reported that he had inflammation in his right knee. He first treated for the incident on 11-18-13 and was diagnosed with bursitis. Report filed for Medical Treatment Only.(fluid drained in knee)

The man was installing bit blocks on the head of the continuous miner. While installing a bit block he struck his hand with a hammer.

The man was roof bolting in a working place. He re-positioned the machine and was preparing to continue bolting. A piece of rock fell between the supports and struck him on the head.

The man was lifting a rub rail at a belt drive when he felt a pain in his lower abdomen. The man continued to work regular duty until 11-25-13 at which time he went on restricted duty.

The employee was exiting a scoop operator's compartment. While twisting his body when exiting he felt a pain in his knee.

Injured person was walking in #4 entry when he tripped on an offset in bottom. As he was falling he put out his left arm to break the fall. He felt pain in left elbow. He has been diagnosed with a left elbow sprain. Treatment will be physical therapy.

The man was inserting a roof bolt in the roof. The bolt was bent to be able to insert into the roof. While pushing on the bolt to straighten it, the bolt spun around causing the man's wrist to twist.

The man was kneeling and shoveling wet coal onto the belt when he developed a pain in his lower left abdomen.

The man was tramming a personnel carrier along the beltline when he struck a bump causing the steering wheel to spin suddenly. The steering wheel knob struck the man on the hand.

The injured employee was standing along the rib near the crosscut. Another employee started to back up a rubber tired personnel carrier. The operator instructed the man to move and attempted to depress the brakes. The operator inadvertently depressed the tram pedal causing the carrier to move backwards and strike the injured person.

A roof fall occurred in the 7 Mains section, # 3 entry at location 7 + 20. The fall was approximately 18 feet long x 16 feet wide x 4 feet high.

The man was assisting the mechanic repair the continuous miner. When he crawled on top of the machine he cut his knee on a metal edge.

The man was tightening the scraper at a belt drive when the bar he was using to tighten the scraper slipped and struck him on the cheek. Report files for Medical Treatment Only (stitches)

The man was roof bolting when a drill steel stuck in the roof. He put his hand over the drill head when the steel fell out of the roof and struck him on the hand.

The man was shoveling coal onto the belt when he felt a pain in his back.

While riding on the personnel carrier on the way out of the mine, the man struck his head against the roof.

A roof fall occurred in the outby area of 4 Mains 40 feet inby survey station 210. The fall was approximately 40 feet long x 18 feet wide x 7 feet high.

The man was pulling the excess roof bolter cable from behind the bolter. He threw the loops of cable behind him. When the bolter began to tram forward the man's leg became entangled in the cable and he fell down.

The man was working in the mine when he felt a pain in his abdomen. He was originally diagnosed and treated for a medical condition without success. On 7-13-12 it was then determined that it was a work related injury.

The man was walking beside the roof bolter when he tripped. He attempted to catch his balance by grabbing the side of the bolter. His hand slid and jammed against the arm of the bolter head boom.

The man was dragging a water pump down the entry when he experienced pain in his leg.

The man was pushing on a accumulation of mud on the mine floor with his foot and felt pain in his knee.

The man was bending a roof bolt when he slipped and fell forward striking his hand against the roof bolting machine.

The man trammed a personnel carrier to the continuous miner, which was broken down, to drop off tools. As began to back up the carrier, he inadvertently had the directional control in the forward position. The carrier began to move forward and the man's leg struck the tail of the miner pushing the man's leg against his chest causing a chest contusion.

The man was walking in the parking lot and slipped and fell.

A roof fall occurred in an intake entry at survey station 182. The fall was approximately 20 feet long x 20 feet wide x 5 feet high.

The man was pulling a pump into a sump area when he developed pain in his right shoulder. The man continued to work until 5-30-12 at which time he reported off work due to the injury.

While attaching a pull behind rock duster to a personnel carrier, the man pinched his thumb between the hitch and the hitch ball.

The man was attaching a supply sled to a scoop. While lifting the tow bar, he felt a pain in his shoulder.

The man was handling rock dust bags when he stated he developed pain in his back.

The man was stacking miner cable in a crosscut when he reported feeling pain in his back. The man continued to work his regular duties until 9-19-11 when he went on restricted duty.

The man was loading belt rollers onto a cart when he felt pain in his lower abdomen.

The man was assisting in the building of a permanent stopping. He tripped on the blocks stacked on the bottom and fell on the blocks, injuring his back.

The man reported pain in his knees. The man continued to perform his regular duties until 9-16-2011 at which time he was placed on restricted duty.

While working as a roof bolter operator, the man stated that he developed pain in his back.

The man was repairing a shuttle car cable reel and when he turned around to get a roll of tape he felt a pain in his knee.

The man pulled down a section of loose rib coal. As he was preparing to roof bolt, an additional piece of rib fell and struck him on the foot.

The man was attempting to separate 2 drill steels that were stuck together. As he was raising and lowering the drill head to loosen the drill steels, he twisted his wrist.

The man was handling belt structure and felt pain in his knee as he twisted. The man continued to work regular duty until 8-3-11 at which time he was placed on restricted duty.

The man was moving a water pump in the section when he felt a pain in his low back while pulling on the pump.

The man was installing a roof bolt when a piece of roof rock fell and struck him on the shoulder.

The man reported that he was walking to the feeder when he felt pain in his low back.

The man was pulling a power cable when he struck his left eyebrow against the sharp edge of a metal roof strap causing a laceration.

The employee was found unresponsive in the belt travelway. The individual was pronounced dead by the coroner. The death was attributed to a heart attack.

The man was climbing off a belt crossover when his foot caught on the step causing him to fall and twist his knee. The man continued to perform his normal duties until 5/25/2011 when he was placed on restricted duty.

The man was hooking a chain between scoop 1 and the feeder. Scoop 2 entered the area and was stopping behind scoop 1. As scoop 2 stopped, it slid in the wet bottom and struck scoop 1, causing it to slide forward and strike the man's arms.

The man was crawling on the mine floor when he felt a pain in his leg. The man's leg was lacerated. He was unsure of what caused the cut. REPORT FILED FOR MEDICAL TREATMENT ONLY (stitches)

The man was building a cement block stopping when a block fell off of the wall and struck the man on the hand.

The man was loosening roof bolting drill steels with a wrench when the wrench slipped and struck the man in the mouth. Report filed for Medical Treatment Only (damaged tooth).

The man was applying B-Bond to a permanent stopping and inadvertently had the material get on his pants. The material soaked through his clothing and caused a chemical burn on his legs.

The man was walking through the section when he struck his head against a roof bolt. The man continued to work until 9-27-10 when he reported off.

The man was tramming a personnel carrier when the steering wheel spun around and the steering wheel knob struck the man on the hand.

The man reported pain in his right knee. The man originally reported his knee injury as non work related. On 8-6-10, further medical evaluations caused a determination that it was a work related injury. This report is being filed subsequent to the injury being determined to being work related.

The man was installing a water pump when the cable became tight and caused the pump to jerk. The man felt a pain in his neck and right shoulder. The man continued to work regular duties until 08/19/2010 at which time he reported off work.

The man was removing a cable off of the hook it was hung on when it fell and struck the man in the mouth damaging a tooth. Report filed for medical treatment only.

The man was lifting material while setting a belt tail and developed pain in his left lower abdomen.

A roof fall was found in the outby return aircourse in the 9 rooms 2 butt area of the mine by the mine examiner. The fall was approximately 820 feet long x 19.5 feet wide x an unknown height.

While moving the roof bolter head forward, the canopy caught against a roof strap. While attempting to release the strap from the canopy, the man caught his fingers between the canopy and canopy extension when the strap came free.

The man was using a come-along winch on a coal chain when the handle slipped and struck the man on the lip requiring stitches.

The EE was inserting a drill bit into a drill steel at the head of the bolter. A piece of rock from around the drill hole fell from the roof striking the man on the hand.

The man was taking down a shuttle car snub when he felt a pain in his back pain.

The man was lifting a bucket of sealant when he felt a pain in his left shoulder.

The man was striking a pipe with a sledge hammer when the hammer deflected and struck the man in the ribs.

A roof fall occurred approx. 3300 feet outby the E-4 section. The fall was 30' long x 25' wide x 4' high.

The man was walking on the travelway to open the travel doors when he struck his hardhat against the roof. The man continued to work full duty at his regular job until January 22, 2010 when he reported off.

The man was carrying a bag of B bond when he felt a pain in his back.

EE was moving a miner cable when he felt a pain in his shoulder.

The man was putting the drill steel in the roof bolter chuck when the drill steel broke and struck the man on the hand.

EE developed pain in his knee while crawling through the section. The EE worked until 11/02/09 when he reported off.

The man was operating a personnel carrier when it struck a bump causing the steering wheel to spin and twist man's wrist. The man worked regular duty until 10-16-09 when he reported off.

While drilling a roof bolt hole a piece of rock got struck on the drill steel. While breaking the rock off the drill steel a piece of rock flew and struck man on eyebrow.

The man was repairing a shuttle car when he twisted and felt a pain in his abdomen.

The man was roof bolting when a piece of scale fell and struck man on upper lip. Report filed for medical treatment only (stitches).

When straightening a roof bolt, it slipped and the plate hit him in the mouth. He received a cut lip and chipped tooth.

Bent over and picked up a cement block for a wall and felt pain in his back.

EE felt pain in his lower back when shoveling belt.

Employee was pulling a loose piece of rock draw with a rock bar, when the rock hit him on his right leg.

The man was installing a bottom roller in the conveyor belt when he felt a pain in his right shoulder. The man continued to work regular duty until 11/10/2010 when he reported off for surgery.

Employee was burning with a torch when a sprak went in his ear. He continued working until 7/24/09 when he missed a day of work.

A roorf fall 9' W x 540' L x undetermined H fell at 12:30 AM in 9 #3 rooms section. The fall does not impede ventilation or travel.

Employee was tightening a bolt to the miner panel when he felt pain in his left shoulder.

A roof fall 19' w x 760" L x undetermined H was found in the return of 9 rooms from 2 xcross cuts outby station 6035 to 1 cross cut outby station 6229. Does not impede ventilation or travel

Employee felt a sharp pain on right side of his groin area while loading posts on the scoop bucket. Original surgery postponed & employee missed no work. Employee will begin missing work for surgery 11/13/2009.

A bolter drill steel popped out of the drill head, bounced back, striking him in his face.

When using a sledge hammer employee got a piece of metal in his right eye. The piece flew under his glasses.

Employee injured his back when lifting and moving posts to set along rib.

The employee's shuttle car snub came loose striking his head.

The employee was roof bolting and drilling a hole when piece of rock (2' around x 5"-6" thick) fell striking his left knee.

Employee stepped in a hole and twisted his leg & hip.

A roof fall 40' L X 19' W X 7' H was found at 8:00 AM at 9 #4 butt station 4627 X cut 3-4. The fall does ot impede ventilation or travel.

Employee was opening door, slipped and lost balance and fell on bit buckets, causing injury to ribs.

EE stepped outside the side door of shop, twisted ankle stepping off a step.

While loading bolter supplies onto scoop bucket the employee twisted his back.

A roof fall was found in 9 mains-#5 Butt-#3 Entry approx 40' inby station #5190. The fall was approx 6' high, 19' wide and 40' long. The area was bolted with 6' fully grouted torque tensioned bolts. The area is dangered off. The fall does not impede ventilation or travel.

The employee slipped and fell backwards when pulling a discharge line injuring his back. He continued working until 6/1/09 when he had surgery.

The employee was operating a shuttle car down grade toward the feeder when a piece of rib (4.5'L x 2'W x 0"-12"T with a 2" rock binder) fell on his leg. The employee reported the piece became wedged between the rib and his chest before he got the machine stopped. He received bruised across his chest and legs.

A roof fall was found in #4 butt of 7 rooms 50' inby station 4742. 160'Long x 19.5 wide x undetermined height. The area was bolted with 6' fully grouted torque tension bolts. The area is dangered off and does not impede ventilation or travel.

A bolter drill steel broke striking the employee on his left temple requiring a couple sutures.

A bolter drill steel struck the employee's left hand when putting the drill steel in the chuck.

When repairing a 6" water line coupler, the coupler broke striking the employees hand. He received lacerations on two fingers requiring sutures.

A roof fall occurred in 9 rooms section #5 butt 10' inby section #5297. Fall was 18' w x an undetermined height x 200' long. Fell from 3-4 entry to #7 entry. 3-4 was bolted on 3x4 with straps in the intersection & 4 8' resin bolts. 5,6,7 entries were bolted on 4x4 with 6' torque tension bolts & 4 8' resin bolts in every intersection.

The EE was straightening a 6' torgue tensioned bolt and the bolt spun around, injuring the EE's left shoulder.

A roof fall was found 80' L x 19' W x undetermined H at 90' outby station 2513 in #2 entry along 8 belt. This fall does not impede ventilation or travel.

The employee twisted his back lifting a jack.

Bolter steel bent and flew over striking ee's left thumb.

The employee received a laceration on his thumb and middle finger when a lifting strap slipped off when changing a battery. The laceration on his thumb required 6 stitches.

The employee cut his finger repairing a hawkeye on a shaker screen. The laceration required 4 stitches.

A roof fall 120'L x 19' W x unknown height was found 80' inby station 4636 in #7 entry of 9 rooms #4 blott This fall does not impede ventilation or travel.

The employee pulled (injured) his lower back loading bolts from the scoop bucket to the bolting machine. His first lost time was November 5, 2007

On November 15, 2007 the employee completed an injury report for October 29, 2007 stating his knee hurt him at the end of his shift. He has been off work since 11-02-07. He had MRI read on Nov. 19 which indicated a medial meniscus tear and is scheduled for physical therapy for six weeks.

The employee slipped on a rock and twisted his right knee on 9/26/07. He continued working until 12/19/07 when he visited a surgeon that recommended time away from work.

When taking out drill steels from the roof, two 3" steels fell out striking the employee's hand against the bolter.

A roof fall 18' wide x 30' long x 8' - 10' high was found in an outby area in 9 main rooms #3 6 panel 3 crosscuts inby station 4264 in crosscut 3x4.

The employee injured himself on 8-21-07 moving a rock out of the way by hand. Continued working until 12-13-07 when he had surgery to repair a hernia.

When installing a 6' roof bolt, the bolt spun around striking the employee's tooth.

While bolting a piece of rock broke off hit the canopy (fly swatter) then struck EE's left hand requiring several sutures.

On 5/5/07 a piece of rock (2'x4'x21/2')fell from roof while bolting striking the EE's back & elbow.Cont'd working until 5/15/07 when walking felt pain in back. Xray showed a fracture to his transverse process (small bone in lower back.).

When the scoop operator pulled back the ram, the ee finger got pinched between the ram and bucket of the scoop.

A roof fall 20' wide X 180' long X undetermined height was found in 9 main rooms #4 Butt old working 2 cross cuts inby station 4159. The fall does not impede travel or impair ventilation and was dangered off.

While moving a fire extinguisher, one accidentally went off. Employee breathed some of the powder.

The employee twisted his back while crawling/walking to set the power to the miner.

A roof fall 20' W x 600' L x undetermined H was found in old workings. The fall dows not impede travbel or ventilation and has been dangered off.

Hammering a bolt, when two pieces of metal went through his left glove. The pieces of steel were the size of a pin head. EE was wearing required personal safety glasses and gloves.

When drilling his outside bolt hole a piece of roof coal fell from between the outside bolt and rib striking the employee on his back.

A roof fall 20' w x 60' L x undetermined height was found in an outby area at one crosscut inby station 3309. The fall dows not impede travel or ventilation and was dangered off.

Employee lifted belt structure and felt something pull in his left shoulder.

While pushing a 8' resin bolt into roof a rock approximately 24" square and 2" thick struck his hand and bolt plate causing a cut that required 8 stitches.

Employee jarred his back while driving a golf cart. He continued to work until August 28, 2006. His first missed scheduled work day was Aug. 29. He is scheduled for an MRI on 9/7/06.

The Employee's knee started swelling & bothering him 2 wks ago from extended periods of crawling & kneeling. It kept getting worse until he sought medical treatmens. He is being treated for blood clots in his knee.

A roof fall 20'x20' undetermined height was found in an outby area at station 2851. The fall does not impede travel or ventilation.

Employee got his finger caught between boom and frame of bolter when he raised the drill head to the bolter. Requiring 12 stitches.

Employee injured groin on 6/9/06 lifting a bucket jack. Had surgery on 8/16/06 to repair a hernia.

A roof fall found in an outby inactive area not affecting ventilation or travel. From crosscut outby spad 2904 to spad 2969. The fall is 280'x 19' x undermined height.

Over an extended period of time crawling, the EE had his knee drained.

The employee received a laceration requiring 8 stitches on a sharp piece of metal when working on a shuttle car.

While roof bolting a piece of rock fell from the roof striking the employee on the side of his head causing brush burns, a loose tooth and tingling in his right arm. The tooth was pulled by a dentist.

Employee stepped in a hole and twisted his back. He kept working until 4/7/06 when his disability began.

An original fall 60' x 20' x undetermined height occurred in #7 entry 30' outby spad 3244. The fall continued 150' outby the next day.

When freeing a stuck drill steel, the EE chipped a front tooth.

A roof fall occurred between 8:00am and 10:00am in an intersection of an outby area. 2 crosscuts inby station 2496 in #5 entry of 8 mains.

While carring bolter supplies employee felt pain in his groin. He was diagnosed with a hernia. On 3/7/06 he had surgery to repair it.

While moving a board a piece of rib (2' x 8" x 4") fell pinching 2 fingers between rib and board requiring stitches.

While loading the scoop with dust the employees back started to hurt.

A ROOF FALL WAS FOUND IN AN OUTBY AREA OF 9 MAIN ROOMS #1 BUTT. 20' WIDE X 200' LONG X UNDETERMINED HEIGHT. FROM #2 ENTRY THRU #5 ENTRY IN CROSSCUTS 16 + 60.

At 10:00 am on 12-6-05, a roof fall was found in an outby area of 9 mains, in #6 entry from station 2709 to station 2923. the roof fall was 20' wide x 80' long x unknown height.

Employee threw a 6' bolt on the roof bolter. The bolt bounced striking him in his upper lip. Required 5 stitches outside and 1 stitch inside.

OVER A PERIOD OF TIME FROM CRAWLING AND KNEELING THE EMPLOYEE EXPERIENCED PAIN IN HIS RIGHT KNEE. MRI DONE 11/03/O5.

The ee pulled down a loose piece of rock (24"x36"x4" thickin the (enter triangular with shape) when it hit the bottom the piece flopped over striking his left knee.

WHILE BOLTING, A PIECE OF COAL (12" X 16" X 3") FELL FROM THE ROOF AND HIT THE EMPLOYEE'S RIGHT SHOULDER.

A roof fall 20' wide x 400' long and an undetermined height occurred from #1 entry to #6 entry in A main rooms at 22+00.

A roof fall occurred in an outby area 2 crosscuts inby station 2628 in #4 entry of 8 Mains running across to #8 entry. 20' wide x 260' long x undetermined height.

Roof fall discovered on pre-shift exam. Occured between 9 am and 7:30 pm. Mine was idle. 9 mains section, #7 entry from face at 12 and 00 outby. approx 240' by 20' undeterminable height but above bolt anchorage.

A ROOF FALL OCCURRED SOMETIME BETWEEN 10:30PM AND 2:30 AM IN A OUTBY AREA OF 8 MAINS IN #5 ENTRY. THE FALL WAS 5' HIGH, 191/2 WIDE AND75'LONG.
