STEEL WIRE FOUND ITS WAY INSIDE OF SPARE BOOTS, EE CHANGED BOOTS MIDWAY THROUGH SHIFT DUE TO WET FEET, EE STARTED TO WALK AND WIRE IMPOVED ITSELF INTO EE'S RIGHT HEEL. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 4-26-01.

WHILE GETTING DOWN FROM THE HI LIFT, INJURED FELL FROM DECK TO GROUND 5-7'. AFTER INVESTIGATING THE ACCIDEN, WE ARE BOIT SUR EIF INJURED SLIPPED ON TOP STEP OR PASSED OUT AND FELL. I BELIEVE H E SLIPPED DUE TO THE FACT THE INJURED REMEMBERS FALLING TO THE GROUND TRYING TO CATCH HIMSELF ON THE WAY DOWN.

THE MAN WAS REMOVING THE 3 INCH DISCHARGE LINE FROM A SLURRY PUMP THAT WAS PLUGGED. WHEN HE REMOVED THE DISCHARGE LINE,HOT SOLIDS AND LIQUIDS FLEW OUT AND STRUCK HIM ON THE WRISTS AND FACE.

THE MAN WAS DRILLING A HOLE IN 1/2" PLASTIC SHEETING WHEN THE BIT CUT ON THE PLASTIC. THE DRILL TWISTED AND THE MAN'S FINGER WAS TWISTED QUICKLY.
