January 6, 2015 PA · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer SLIP OR FALL OF PERSON
Bedrock Mines, LP · Over-exertion (Not Elsewhere Classified)

When exiting the loader a gust of wind blew the door closed. While trying to catch the door from closing on him, EE slipped on the stair deck. In an attempt to stop his falling, he reached for a hand hold, straining his left shoulder and bicep. And irritated a nerve in his cervical spine (neck).
