Employee was climbing down the loader ladder when EE's right foot slipped causing EE to fall on EE's right knee.

Employee lost balance when foot slipped off ladder causing him to fall approx 4ft to the ground causing him to hit his head and fracture his ankle.

Fall from scaffolds, walkways, platforms

Employee was walking across the catwalk positioning himself to help in a screen removal. The catwalk slid off the edge, allowing the employee to fall down and into a product chute. The employee did not go to the doctor until 7/24/2009, at this time the injury became MSHA reportable.

A piece of hot slag entered the EE right ear canal after using a torch on a grizzly feeder. EE had surgery on December 16 which made this claim reportable.

Employee was working on tail pulley of a conveyer. The guard that protects this point was flipped up but not secured. The guard flipped down to its closed position striking the ee's fingertip. Tip of left index finger was struck. EE received stitches and finger was splinted for a break.

INJURED WAS CARRYING HIS TOOLBOX IN HIS RIGHT HAND FROM THE OIL STORAGE CONTAINER TO THE WELDING TRUCK WHEN HIS RIGHT FOOT CONTACTED THE CHANNELBASE OF A METAL ROLLER STAND CAUSING HIS RIGHT F OOT AND ANKLE TO TWIST.
