Employee was walking across the catwalk positioning himself to help in a screen removal. The catwalk slid off the edge, allowing the employee to fall down and into a product chute. The employee did not go to the doctor until 7/24/2009, at this time the injury became MSHA reportable.

Employee was working on tail pulley of a conveyer. The guard that protects this point was flipped up but not secured. The guard flipped down to its closed position striking the ee's fingertip. Tip of left index finger was struck. EE received stitches and finger was splinted for a break.

WHEN PLACING SLINGS ON PLATFORM FOR CRANE TO LIFT, PLATFORM TIPPED AND HUNG UP ON HANDRAIL. BECAUSE PLATFORM WAS ON THE GROUND, EE MANUALLY FREED THE PLATFORM FROM THE RAIL CAUSING THE RAIL TO SLIP AND PLATFORM TO FALL PINNING EE'S LEG.
