Fall from ladders
Employee was standing on a stool while thawing frozen air lines on a Cat 777 truck. The stool collapsed, causing the employee to fall bumping his head on the ground. The stool was a 2 step stepladder, in which a rivet holding the side brace broke, causing it to collapse
