While straightening a roof bolt the roof bolt spun cause a pain in the left wrist of the employee

Caught in, under or between a moving and a stationary object

While making a left turn with his shuttle car he raised his left arm to push a run through check out of his line of sight. At that time the car slid sideways pinching his left arm between the rib and the shuttle car.

Struck by falling object

While changing drill steels a piece of rock slid down the drill steel pinching his little finger between the drill steels causing a laceration and possible broken finger.

Caught in, under or between a moving and a stationary object

While sliding a ""C"" channel to the front of the bolter, employee had his fingers in the slot of the channel, the channel slid over a crib block amputating the tip of his right hand ring finger.

Accident type, without injuries

While mining an auger hole (uncharted) was exposed with the continuous miner a large amount of water came out of the exposed hole. Apx 18,000 gallons.
