Employee was putting a fork on the forklift when they lost control of the fork and it fell striking their right foot. The blow caused a small cut on the top of the right foot just above the steel toe on their boot

Shredder hopper was sitting on floor. Employee was preparing to hook chains to the hopper and loader. When the employee put EE's weight on the hopper, it moved causing the employee to hit loader bucket with EE's back. The rim of the hopper struck the employee in the stomach and rib area. The employee's back and ribs were bruised.

Employee was cleaning Carbon Testing Machine when her eyes began to burn. She washed her eyes with eye wash solution. It seemed to help. Employee worked the remainder of her shift and on her way home her eyes continued burning so she decided to go to the doctor. Employee was back at work on Tuesday which was her next regular scheduled day to work.

EE WAS WORKING ON THE TRUCK LOADING PLATFORM. THIS AREA IS LESS TRAVELED THAN ANY OTHER AREA OF THE PLANT. THE STEEL H BEAMS ARE A PRIME LOCATION FOR WASPS TO BUILD NESTS. AS HE WAS PERFORM ING HIS REGULAR DUTIES IN THE AREA WHERE A NEST WAS LOCATED, HE WAS STUNG BY WASPS ON THE CHEEK & THE BACK.
EE WAS WALKING THROUGH THE PLANT, HE STEPPED AND SLIPPED ON A WET SPOT ON THE FLOOR. WATER/WET SPOT WAS OBSCURED BY DUST. THE INCIDENT OCCURRED IN THE 53 MILL AREA.
