Employee was burned by hot coolant when they were attempting to check fluid levels. Employee was hit in the chest/neck/arms/head with hot coolant. First aid was attempted onsite but determined employee needed to go to ER.

The employee was welding a wear plate inside the bed of a haul truck. After completing a weld, the employee chipped off slag and was visually inspecting the weld when a hot piece of slag rebounded and struck the employee in the eye, resulting in an eye injury that required medical evaluation

Employee was operating the CAT D9 dozer. When getting out of the cab of the dozer, employee thought the door was in the locker, open position, and it was not. When EE pulled on the door to get out of the dozer, the door hit EE in the mouth and broke a front, upper tooth at the gumline.

IE was unloading boxes from the tailgate of a pickup truck and carrying them into a Connex storage container. As IE stepped up into the container, EE's foot slipped and EE's right leg came down hard onto the ground causing a sprain to EE's right knee.

While exiting a water truck, the employee missed the second rung from the top of the ladder and fell approximately three feet to the ground. During the fall, the employee attempted to regain balance by grabbing the door, which caused EE's body to shift to the right. Upon impact, EE landed on EE's right elbow and reported pain.

Removing track adjuster from 349F excavator, removed mounting bolts and let adjuster down onto roller, went to move adjuster to let fall out of frame, adjuster fell and crushed middle finger on right hand between track and adjuster. used pry bar and lift adjuster off of hand, remove glove and found bad laceration to middle finger.

Ee was picking up a porta band saw. EE grabbed the trigger with EE's right hand and then reached to the blade with left hand. EE didn't realize the safety was not engaged. While picking it up, EE depressed the trigger and activated the saw. The saw blade caught EE's glove, pulling EE's hand into the blade. Ee sustained a cut to EE's left index finger requiring stitches to close.

According to technician, all required PPE was worn at the time and flushing of eye was ineffective. No further treatment has been recommended since removal of metal sliver and employee was back to work.

Employee was dismounting dozer in the wash bay. The foot on the dozer arm slipped and they fell backwards getting right leg hung up between the track frame and the dozer arm for a short time. After leg came free, employee fell to the ground. X-ray revealed a fracture at the end of the femur bone where the ligament attaches above the right knee.

Oiler was preparing to fuel 527 Loader. When EE turned to the left, right knee "gave out" and EE collapsed to the ground. EE was examined by a physician and diagnosed with a comminuted femur fracture and torn ligaments which will likely require surgery to repair.

EE pulled down on a Chain Spreader tool handle. The handle suddenly released downward straining employee's shoulder. Employee saw a physician on 9/9 and released back to work as tolerated. On days off the employee saw an Orthopedic Physician 9/30 and was released restricted. The employee returned to next scheduled shift restricted on 10/5 making this a reportable case at that time.

While lifting and turning to place a partially filled 5 gallon bucket of sample material into the back of a pickup, employee strained lower back. This has been a first aid case until 7/15/2020 when employee was prescribed Physical Therapy treatments. Employee has not missed any work to date.

While pressing an apron feeder track pin into place with a hydraulic track press. The employee chose to add a small 3-pound sledgehammer at the end of the pin to steady it. Once pressure was applied, the sledgehammer was ejected from its location and struck the employee in the chin fracturing one tooth.

While lifting a 50 pound bag over their head to put into a splitting bin, the employee felt a sharp pain in the neck and back just to the right of the spine. This injury occurred on 4/23/2020, however the employee was reexamined and prescribed physical therapy which began on 6/9/2020, which made the event reportable at that time.

Employee was manually moving a 6 inch poly pipe on 10/2/19. When the employee put the poly pipe down EE felt a sharp pain in EE's back. Due to minimal improvement in condition, the employee started Physical Therapy on 12/12/2019 which changed the status of the event from a First Aid event to a reportable Medical Treatment Case.

While operating a D10T Dozer to build a new access road, the employee tracked over a topsail berm upon reaching the bottom of an embankment. As the dozer crested over the top of the berm, the tracks of the dozer contacted the ground with enough force that the seatbelt restrained operator sustained a lower back strain.

Another tech and myself started to disassemble the accumulator and failed to bleed off the nitrogen before starting disassembly. I was holding the ratchet on the end cap while ee ran the ratchet. When the end cap came to the end of its threads the pressure that was still in the accumulator blew the end cap into my hand.

During the day, an employee stated EE was experiencing pain in EE's right, upper abdomen. EE could not remember any event that brought on the pain. After seeing a doctor the next day, EE was diagnosed with a hernia.

Operators were winterizing an eight inch poly line. Air was blown through the pipe and was exiting a 2" valve on the other end. An operator ensured that all air had exited the 2" valve. An operator disconnected a 4" quick coupler and the air that was still in the pipe knocked the operator to the ground. The medical determination was a sprained MCL. The investigation is on going.

After priming a blasthole, the ee stepped back to kick some drill cuttings away to bury the down line. When he stepped back he fell into a blasthole that had been re-drilled. He experienced pain in right knee but did not know it was a serious injury.
