Employee was cutting metal on the metal rack, after the metal was cut through the cut end came up and contacted the employee's lower lip. The lower lip was bruised, and two lower front teeth were chipped.

Worker was splitting a dozer track, when the track came apart EE's left ring finger was pinched in-between track pads causing injury to finger which required sutures and the tip of the bone was fractured

Operator jumped off equipment from approx. 3 ft after incident. Felt pain in right knee from twisting motion when contacted ground

Employee was changing out the water jug on the water dispenser when the bottle made contact with the employees chin causing a tooth cap to fall off.

Employee was wire stripping using a Klever Kutter when the plastic edge of the tool broke causing the tool to slip and lacerate the palm of the employees left hand. Employee was taken to the Urgent Care and received 5 stitches with a return to Work as Tolerated.

Employee was tightening bolts on aa heat exchanger when the wrench slipped off the bolt causing discomfort in left shoulder

Tire technician was blowing debris, mud, dirt and rocks off rim base for truck 415 with compressed air and got a piece of metal in right eye. According to technician, all required PPE was worn at the time and flushing of eye was ineffective. No further treatment has been recommended since removal of metal sliver and employee was back to work.

Driller Assistant was using a propane weed burner torch to thaw a frozen valve on the bottom of a water tank when the Driller Assistant experienced frostbite on the ungloved hand and fingers that were in contact with the stem of the torch.

Plant Tech was walking a leach pad doing routine checks when they tripped on a dripper hose falling onto their knees

Maintenance worker was changing out a broken mirror on a haul truck when the wrench slipped off a bolt causing the left index finger to come into contact with the mirror resulting in a laceration that required 4 sutures

Operator was driving Haul Truck in the Portland Ridge pit when they hit several bumps in a row causing discomfort to lower back, right knee and right foot.

Employee was dismounting dozer in the wash bay. The foot on the dozer arm slipped and they fell backwards getting right leg hung up between the track frame and the dozer arm for a short time. After leg came free, employee fell to the ground. X-ray revealed a fracture at the end of the femur bone where the ligament attaches above the right knee.

Employee was walking with a short flathead shovel into the tool room at the process plant when EE tripped over a cushion mat on the floor, while trying to catch themselves the shovel handle made contact to the front teeth causing the left one to chip in half.

Employee was removing a trailer receiver from a light duty pickup, when EE pulled it out EE pinched EE's right ring finger between the hitch and the trailer EE just disconnected. X-RAY of the finger showed a small fracture towards the tip

Oiler was preparing to fuel 527 Loader. When EE turned to the left, right knee "gave out" and EE collapsed to the ground. EE was examined by a physician and diagnosed with a comminuted femur fracture and torn ligaments which will likely require surgery to repair.

EE pulled down on a Chain Spreader tool handle. The handle suddenly released downward straining employee's shoulder. Employee saw a physician on 9/9 and released back to work as tolerated. On days off the employee saw an Orthopedic Physician 9/30 and was released restricted. The employee returned to next scheduled shift restricted on 10/5 making this a reportable case at that time.

While lifting and turning to place a partially filled 5 gallon bucket of sample material into the back of a pickup, employee strained lower back. This has been a first aid case until 7/15/2020 when employee was prescribed Physical Therapy treatments. Employee has not missed any work to date.

While pressing an apron feeder track pin into place with a hydraulic track press. The employee chose to add a small 3-pound sledgehammer at the end of the pin to steady it. Once pressure was applied, the sledgehammer was ejected from its location and struck the employee in the chin fracturing one tooth.

While lifting a 50 pound bag over their head to put into a splitting bin, the employee felt a sharp pain in the neck and back just to the right of the spine. This injury occurred on 4/23/2020, however the employee was reexamined and prescribed physical therapy which began on 6/9/2020, which made the event reportable at that time.

Employee was manually moving a 6 inch poly pipe on 10/2/19. When the employee put the poly pipe down EE felt a sharp pain in EE's back. Due to minimal improvement in condition, the employee started Physical Therapy on 12/12/2019 which changed the status of the event from a First Aid event to a reportable Medical Treatment Case.

Employee was bent over working on a piece of pipe. When the employee stood up they felt a sharp "Pinch"/pain in the lower back.

While operating a D10T Dozer to build a new access road, the employee tracked over a topsail berm upon reaching the bottom of an embankment. As the dozer crested over the top of the berm, the tracks of the dozer contacted the ground with enough force that the seatbelt restrained operator sustained a lower back strain.

A haul truck operator slipped on icy ground injuring their right shoulder and arm.

While offloading a drill tool, the driller caught right pinkie finger in a pinch point. The driller received a laceration to the finger requiring sutures.

Another tech and myself started to disassemble the accumulator and failed to bleed off the nitrogen before starting disassembly. I was holding the ratchet on the end cap while ee ran the ratchet. When the end cap came to the end of its threads the pressure that was still in the accumulator blew the end cap into my hand.

The employee was holding a bolt head from the bottom of the cutting edge so another employee could remove the nut from the bolt with the air wrench. The employee lost control of the head bolt causing it to spin lacerating EE's right index finger.

The dozer operator backed over a large rock. Employee was jarred in the cab when the dozer cleared the rock.

The employee pinched left index finger between two pieces of 12" pipe amputating the tip of finger.

An employee slipped and fell on icy ground. Employee worked employee's scheduled shifts and full duty until March 5th when employee did not report to employee's scheduled shift.

A driller walking on the pattern at night stepped into a drill hole and twisted EE's knee. After medical treatment and observation, it was determined EE needed surgery. EE missed EE's next scheduled shift on March 8th.

Employee was removing guard from the belly pan of a 16M Motor Grader. A chain hoist and jack were being used to support the guard. Employee was guiding it out when it slipped off the jack smashing left thumb between the guard and the concrete floor.

The haul truck operator hit a soft spot in a down ramp while in EE's loaded truck. The action produced pain in EE's lower back. EE was not able to return to EE's next scheduled shift.

During the day, an employee stated EE was experiencing pain in EE's right, upper abdomen. EE could not remember any event that brought on the pain. After seeing a doctor the next day, EE was diagnosed with a hernia.

Employee changed direction from forward to reverse in a front-end loader. Employee experienced pain in lower back. On February 11th, sought medical treatment from a Chiropractor.

Truck Operator was exiting the transport van and slipped on the running board which had iced up from the snowy weather conditions and fell to the ground.

A front-end loader operator hit a soft spot on the bench while operating the loader. The loader dipped sharply to the left throwing him sideways in the cab. He was wearing his seat belt. The motion aggravated a pre-existing condition in his lower back. He was assessed at a local hospital. Due to his pain level, he was not able to make his next scheduled shift.

Haul truck operator was washing his front window, the operator was standing on the catwalk at the top of the stairs. The operator lost his footing falling down the stairs; the operator attempted to catch himself on the hand railing above the radiator and impacted his left arm on the radiator housing.

Crusher Operator was lifting impact rails about 25lbs over his head to co-workers. After lifting the fifth rail the operator felt pain in his right shoulder that started to radiate down his right arm to fingers. The operator returned to next scheduled shift, after a few months shoulder did not heal operator was seen by doctor whom suggested surgery.

Equipment operator was closing the door on the loader when he caught his right pinkie finger between the door and door frame. The equipment operator received sutures as a result.

Drill Operator was pulling the pull rope to start a 5 HP generator. The operator felt discomfort in his back and reported it to his supervisor. The Drill Operator went to chiropractor for adjustment.

Crusher Operator was dismounting the frame of a Tertiary Crusher when his foot slipped and his other foot hung up on the arm guard of the cone causing his knee to hyper-extend.

On the nightshift of 7-31-2014 the Haul truck operator finished fueling his truck and accessed his haul truck ladder and claimed to hurt his back the HT Operator reported the incident 8-4-2014 AM. The HT operator was not released to return to work after Appt. on 8-7-2014.

Operator was walking on unconsolidated material when a rock shifted under his foot he reached down to balance himself and cut his right hand on a sharp rock requiring three stitches.

Craftsman was shoveling snow and lifted, twisted and turned simultaneously and caused pain to his lower back. Went to chiropractor for treatment.

Pad Operator 2 was operating the backhoe pulling pipe off of the leach pad and started to notice tightness in his right upper part of back. Operator sought treatment at chiropractor.

Operator was changing loader edges and was removing the edge bolts. One of the edge bolts fell through the grove and EE held it back up with his left hand and when he used the air wrench the bolt caught his glove and hand and lacerated his thumb pad the operator required stitches.

Supply Clerk unlocked the double doors to the warehouse filter storage building pushed the lever down, pulled the lever to open the door and the handle broke tearing through his glove and lacerating his right index finger.

Operator was descending a fixed ladder when left foot stuck in the ladder rung and twisted his knee as he reached with his right leg for the next rung. The operator suffered slight tendon tear and started physical therapy on his left knee 1-27-2014.

Production loader operator had his right foot sitting in an idle position while operating a loader with his ankle rolled to the side, it felt like it was starting to fall asleep. He proceeded to walk around the loader to relieve the pain and was unable to.

Eaton construction was using a guzzler truck to excavate material next to the Assay Lab. An EE was using the wand and handed it to IE and IE was unable to keep it from passing over his left foot. The high pressure cut a hole in his boot and foot about 3/8" in diameter. IE was unable to return to his next scheduled shift.

Plant Operator was adjusting a pipe manifold over the top a protective rubber mat with the help of a co-worker. When he and the co-worker lifted the pipe he felt a pop in his neck and shoulder. The operator did not feel medical aid was required at the time, the operator then sought medical aid on 8-30-2013 and 9-3-2013.

Employee was assisting a tag line when he hit a bracing bar on a stationary belt with his left knee and the knee hyper extended. He did not seek a doctor until 8-30-2013. The doctor has recommended physical therapy.

Drill Helper was attaching alligator clips to 12 volt battery to power a diesel fuel transfer pump, when he did so he felt a shock and pain to his right arm.

Exploration drill helper moving a pipe back to the rack stand. Helper did not use the proper method to handle the pipe twisted and turned instead of lifting and walking with the pipe. The helper has experienced severe back pain and will be evaluated medically before return to work.

Operator was tramming a track drill on a snow covered bench when he hit a loader tire rut approximately 8" deep. When the drill came out of the rut the front tracks went up into the air and slammed to the ground jarring the operator while he sat in his seat.

Employee was walking down traction type plastic on a slight grade and he felt his knee give out. It was determined to be a tear to ACL and Meniscus, worked until surgery on 12-6-2012.

Installing Rolling Steel door, putting curtain of door into guides and employee's finger got pinched between the curtain and the wall. This cut the tip of his middle finger on his right hand.

Operator was walking from his haul truck to the shop. The operator slipped on the ice and fell to the same level. The operator was assessed at the emergency room with a fractured left humorous bone.

Operator was cutting a piece of flat cardboard with a pocket knife, the knife did not cut all the way through in an area. The operator then pushed harder with the knife causing it to slip and cut his left hand at the base of his thumb. The operator was transported to the ER room where he received (2)two sutures.

Operator was changing GET using deadblow hammer and punch when a piece of the GET flew off and penetrated operator's left leg above the knee. The operator is scheduled to have the metal removed on 7-27-2011. The operator has not missed any work to date.

Employee was working on a blast pattern when he fell for no apparent reason. The supervisor recognized the signs of a stroke and quickly transported the employee to the mine first-aid station for further evaluation. The employee was later treated for a minor stroke at a hospital. We do not believe that the employee's work environment played a part in the onset of this illness.

Employee lifted two bundles of lathe and as he was doing that felt and heard a pop in his shoulder. The employee did not see improvement went to doctor on 5-3 and the doctor diagnosed surgery needed on 6-1-2011.

Haul truck was being loaded with spent material, there was some frozen chunks in the material. The loader dumped the bucket of material in the truck box and a large piece of ice hit the offside of the haul truck jarring the truck operator. When the operator was jarred he experienced back pain, the operator was recently released by a physician for a back injury unrelated to work.

Operators were winterizing an eight inch poly line. Air was blown through the pipe and was exiting a 2" valve on the other end. An operator ensured that all air had exited the 2" valve. An operator disconnected a 4" quick coupler and the air that was still in the pipe knocked the operator to the ground. The medical determination was a sprained MCL. The investigation is on going.

Mechanic was removing defuser from hydraulic filter with hydraulic filter still in the housing. Pulled the defuser out with pliers and the whole filter came out, pinching the middle finger between filter and housing. Mechanic received five sutures in middle finger.

Employee was sliding a large rock off of a dozer arm. When the rock began to fall he twisted his body to get out of the way. He experienced pain in his left side middle back.

No specific incident occurred. A small fragment of steel became inbedded in the employees eye while performing routine clean up at the crusher. Injury was determined to be work related by Workers Compensation on 07/06/2010.

Employee was digging a trench with a spade. Impact from the hard ground while digging caused pain in left shoulder.

Employee was adjusting a newly installed door in the crusher building. Adjustment was above head level causing employee to look up while adjusting. A gust of wind blew a foreign object into his eye.

Employee was walking along a stream bank when he slipped on moist vegetation. When he attempted to catch his fall with his hand, his hand contacted a tree branch, causing a laceration.

Employee was washing a 2 liter glass beaker when it broke, resulting in a laceration to right index finger that required 5 stiches

Employee was using an air impact wrench to loosen a bolt on a cutting edge. Bolt came loose sooner than expected. Torque of impact wrench made wrench rotate up striking the person on the lip. Laceration to lip required stitches.

Employee experienced pain in left arm due to repetetive motion. Employee was seen by a physician. No medical treatment received, but was put on restricted work activity.

Employee was carrying a framed picture to his vehicle, when the glass in the frame broke and fell on his hand. Laceration to left hand, required 10 stitches.

Sand truck operator was sanding the haul road into the Deep Portland pit. Approximately mid way down the ramp, the operator applied the brakes to slow down and they did not slow the truck down. All attempts to stop the truck failed. Operator steered the truck into a pile of material at the bottom of the ramp. Operator received 2 stitches on chin for cosmetic purposes.

Employee was stretching out a 3" water hose and handing the nozzle to another person. As the hose unwound, the nozzle rotated and hit the employee on the lip. Two stitches required to cloase laceration. Dental work will be required to repair chipped tooth.

Operator was moving drill parallel to a cutbank when the material on the crest gaveway. Drill tipped over on it's side and fell into trench. Operator was ejected out of the drill through the window and came to rest when he hit the drill mast. Operator received two broken ribs.

Employee was sitting in a Caterpillar 777F 100T haul truck getting loaded by a Caterpillar 992G loader. With the full bucket of the loader over the box of truck, the loader operator pushed the wrong lever causing the bucket to fall onto box of truck. Truck driver was tossed around in cab causing strained neck muscles and a sore thumb.

Employee was helping to lift up a hood on a straight truck when he planted his right foot on uneven ground causing his knee to pop and cave in. An MRI revealed a torn ACL and the employee was advised on 09/05/2007 that surgery was required to repair the ACL. A date for the surgery has not yet been scheduled.

Employee was using a vise grip, holding a punch to pound out a u-joint on a pick up truck driveshaft. Hammer glanced off of punch and hit left index finger. X-ray revealed a fractured bone.

Employee was ascending two steps to entrance of Warehouse office, when she experienced pain in the back of her right knee. Doctor visit revealed pulled hamstring. Employee unable to work next scheduled shift.

Employee was picking up the end of a tow strap when he felt a pull in his lower back followed by slight numbness in right leg. Doctor visit revealed a pulled muscle in lower back.

Employee was kicking in drill cuttings into a blast hole to get to designed hole depth when he felt something "pop" in his left knee. An MRI reveaqled torn meniscus and the employee was advised on 03/29/07 that orthoscopic surgery was needed. The surgery has been scheduled for 04/06/2007.

Employee was moving a portable fuel tank to a sump area. While in the process of connecting fuel lines his face struck against a piece of tin sticking up above the tank. Laceration to face requiring stitches.

Employee was putting oil in a Cat 16G Motorgrader. when he turned to walk on tandem housing. When he placed his foot on the tire he slipped and fell to the ground. Weather conditons were snowing. Slight oil spill may have contributed to the accident. Injury consists 3 fractured ribs.

Employee was cutting a piece of plastic liner with an exacto knife when his left hand. slipped causing the knife to come away from the plastic. Momentum of the hand and knife continued on cutting his right forearm. Laceration to right forearm.

Employee was loading haul trucks. loader tire went into a soft spot on floor, employee experienced pain in his lower back as a result.

Employee was lifting and handing off an anti-spin mechanism out of a cone crusher, when he experienced sharp pain in lower back, buttocks, and legs. Doctors report reveals bulging disc in lower back. Unable to work next scheduled shift on 3-17-06

Employee was exiting the cab of a drill. He was walking on a platform when his left ankle was caught between the cab and a deck wrench lying on the deck. Employee twisted his ankle causing a sprain.

Employee was dismounting from a 100T haul truck. When he stepped down on the ground with his right foot, he felt a pop in his right knee. Shortly after, the knee began to ache and swell.

Employee was hitting a loader tooth with a sledgehammer to loosen it up for removal. Small piece of steel from tooth broke off and impaled itself in employees right leg. Steel removed and leg stitched up.

Employee was grinding on a piece of channel iron, with safety glasses and a welders hood on. Piece of metal flew into his left eye. Metal was removed by flushing and a prescription antibiotic was applied to prevent infection.

Employee was priming blastholes on a blast pattern that had received 3 to 4 inches of fresh snow the night before. After priming a blasthole, the ee stepped back to kick some drill cuttings away to bury the down line. When he stepped back he fell into a blasthole that had been re-drilled. He experienced pain in right knee but did not know it was a serious injury.

Employee was walking to a pick up when he slipped on snow packed ground spraining his ankle.

Employee was lifting and carrying a loader tooth (992G) from bed of p/u to bucket. After installing tooth on shank and straightening up, he noticed pain in his lower left side of back. Was seen by a chiropractor.

Employee was beginning to climb a ladder on a 100T haul truck. As he stepped on the first step and pulled himself up he felt pain in his lower back. Due to pre-existing non-work related injuries & illnesses the injury was reviewed and determined to be work related on 10-27-04.

EMPLOYEE BENT OVER TO PICK UP A DOZER TRUNNION BALL FROM THE FLOOR. WHILE LIFTING BALL ONTO SHOP TABLE HE STRAINED LOWER BACK MUSCLES. WAS UNABLE TO RETURN FOR REGULAR WORK SCHEDULE ON 4/25/04.

NO SPECIFIC INCIDENT OCCURRED TO CAUSE INJURY. EMPLOYEE COMPLAINED OF A SORE NECK & SHOULDER AS A RESULT OF OPERATING EQUIPMENT OVER A LONG PERIOD OF TIME. TURNING HEAD TO LOOK OVER SHOULDER WHEN BACKING IS A CONTRIBUTING FACTOR. EMPLOYEE DID NOT SEEK MEDICAL TREATMENT INTIL 03-31-04.

EE was checking pipe line connections on leach pad #4 when he slipped & fell on some ice. He immediately felt sharp pain in his lower back. Did not seek medical treatment until 03/15/04.

REMOVING A SCREEN FROM A SCREEN DECK, AS HE REMOVED SCREEN HIS HAND STRUCK AGAINST A BRACKET ATTACHED TO THE SIDE OF THE SCREEN DECK, CAUSING A LACERATION TO THE BACK OF HIS HAND. STITCHES.

EE WAS WALKING TO A HYDRAULIC UNIT TO ADJUST PRESSURE WHEN HE TRIPPED ON A ROCK. THIS CAUSED HIM TO FALL INTO STEP AT THE HYDRAULIC UNIT. IMMEDIATE INJURY WAS A LACERATION TO HIS LT. LEG. EE STATES HE "WOKE UP THE NEXT DAY & FELT PAIN IN MIDDLE OF BACK SHOULDER. HE WAS SEEN BY A CHIROPRACTOR 8/13/03. REPORTED BACK PAIN 8/14/03.

EMPLOYEE WAS PLACING PIN FLAGS INTO THE GROUND. AS HE WAS STRAIGHTENING UP FROM PLACING A FLAG HE NOTICED A PAIN IN HIS LOWER LEFT SIDE OF BACK. WITHIN 30 MINUTES HIS LEFT FOOT BEGAN TO TINGLE AND BECOME NUMB. HIS CALF MUSCLES BECAME TIGHT WHEN WALKING. HE WAS UNABLE TO WORK HIS SCHEDULED SHIFT ON 08/04/03.

EE STATES ROUGH ROADS MAY CAUSE THE SORENESS TO BACK AND NECK. AFTER HITTING BUMPS IN THE ROAD I HAD PAIN IN MY BACK UPPER AND LOWER EE WAS SEEN BY A CHIROPRACTOR.

EMPLOYEE WAS DRIVING A 100T HAUL TRUCK WHEN HE HIT A POTHOLE IN THE HAUL ROAD. HE BEGAN EXPERIENCING PAIN IN HIS BACK AS A RESULT. DUE TO AN EARLIER NONWORK RELATED INJURY HE EXPERIENCED, IT WAS QUESTIONABLE BY WORKER'S COMPENSATION AS TO WHETHER THE INJURY WAS WORK RELATED. DOCTOR'S VISITS AND CONSULTATION DETERMINED ON 5/29/03 THE INJURY TO BE WORK RELATED.

NO SPECIFIC INCIDENT OCCURRED TO CAUSE INJURY, SORE BACK IS A RESULT OF REPETITIVE LIFTING AND CARRYING SAMPLE BAGS OF MATERIAL.

EE WAS SHOVELING MATERIAL WITH HELP FROM ANOTHER PERSON. SHOVEL TIP FROM OTHER PERSON'S SHOVEL STRUCK EE'S THUMB CAUSING A LACERATION REQUIRING 5 STITCHES.

EE WAS LIFTING AND MOVING A STEEL PLATE FROM ONE SAWHORSE TO ANOTHER WHEN HE FELT A SHARP PAIN IN THE RIGHT SIDE OF HIS LOWER BACK.

EMPLOYEE WAS CUTTING A DRIPPER LINE WITH AN EXACTO KNIFE. LINE WAS PARTIALLY CUT AND BEGAN SPRAYING SOLUTION. EE ATTEMPTED TO FINISH CUTTING LINE WHEN HE CUT HIS LEFT INDEX FINGER. STITCHES REQUIRE TO CLOSE LACERATION.

EE WAS OPERATING IR DM-45 DRILL. WHILE COLLECTING HOLE SAMPLES AND RETRIEVING SAMPLE PAN, EE NOTICED A SHARP PAIN IN HIS BACK. HE WAS SEEN BY A DR. AND BEGAN PHYSICAL THERAPY TREATMENT ON 11-0 1-01.

EE WAS WORKING AT THE CRUSHER, ASSISTING VULCANIZER SPLICE ON CONVEYOR BELT. HE WAS CLAMPING TWO 2X4 PIECES OF LUMBER ONTO THE BELT. ONE OF THE BOARDS POPPED UP HITTING HIM IN THE MOUTH. LACER ATION TO LOWER LIP. ONE 1 STITCH REQUIRED.

EE WAS IN THE WELDING BAY WHEN HIS EYE BAGAN TO HURT. EYE BEGAN WATERING AND WAS SENSITIVE TO LIGHT. OPTOMETRIST REMOVE SLIVER OF METAL EMBEDDED IN EYE.

EE WAS ATTEMPTING TO LINE UP A DRILL BIT SO A WRENCH WOULD FIT ON THE BIT. WRENCHED DROPPED DOWN ONTO BIT WHILE HIS HAND WAS STILL IN PLACE. RIGHT INDEX FINGER PINCHED. STITCHES REQUIRED FOR L ACERATION. NO FRACTURED BONES.

EMPLOYEE WAS CLIMBING INTO A SCREEN DECK TO PERFORM MAINTENANCE. FINE AIRBORNE PARTICLES WERE IN THE AIR FROM THE TOP SCREEN. SMALL PARTICLE BECAME IMBEDDED IN LEFT EYE, REMOVED BY EYE DOCTOR.

EE WAS REPOSTIONING A LARGE HEATER BY PUSHING ON FAN GUARD. FAN GUARD BROKE ALLOWING FINGER TOSLIP THRU GUARD. FAN BLADE AMPUTATED DISTAL END OF RIGHT LITTLE FINGER

EE WAS PERFORMING CLEAN UP BEHIND JAW CRUSHER. ROCK FROM DECK ABOVE FELL AND STRUCK EE IN THE NECK. DOCTOR REPORT REVEALS BRUISED MUSCLES. X-RAYS REVEAL NO BROKEN BONES.

EE PREVIOUSLY HAD SURGERY ON HIS NECK FOR A RUPTURED VERTEBRA. THIS INJURY WAS NOT WORK RELATED. EE RETURNED TO WORK AFTER SURGER AND ON 6-22 COMPLAINED OF A SORE NECK FROM DRIVING HAUL TRUCK. EMPLOYEE WAS DISMOUNTING FROM THE REAR OF A CATEPILLAR 16G MOTORGRADER. HIS RIGHT FOOT BECAME MOMENTARILY CAUGHT IN A BRAKE LINE CAUSING HIM TO LOSE HIS BALANCE AND FALL OFF OF THE MACHINE. EM PLOYEE LANDED ON HIS LEFT ELBOW. PAIN AND SWELLING, AND POSSIBLY FRACTURED ELBOW.

EMPLOYEE WAS CHANGING HAMMER ON DRILL STEEL. WHEN HE MOVED A PIPE IT CAUSED HIS THUMB TO BE CAUGHT BETWEEN THE HAMMER AND THE CAROUSEL ON THE DRILL. LACERATION TO THUMB REQUIRED THREE STITCHES
