EMPLOYEE WAS 5 FT AWAY FROM LIFT AREA WHEN LIFTING EYE ON CONE WEAR LINER BROKE FREE & CHAIN USED TO LIFT WITH STRUCK THE EMPLOYEE KNOCKING OFF HIS HARD HAT & CAUSING A LACERATION ON THE SIDE OF HIS HEAD & SKULL FRACTURE.
