Employee was removing wear edges on a Caterpillar 990 Wheel Loader Bucket. While removing wear edge bolts with air impact wrench, employee was using left hand to hold bolt head in place. The bolt head spun causing a laceration to left middle finger.

Employee was beginning work on hood of Mack truck. Stepped up on bumper, foot slipped and he fell back onto steel bumper of service truck which was parked in front of Mack truck.

Employees were putting belt laces in a conveyor belt. One of the employees was using a hammer and chiesel and a small sliver of the chiesel broke off and went into the other employees lower leg below the knee resulting in the employye having to go to the clinic and have the sliver removed. The chiesel being used was new and did not have any mushroomed edges.

Driver had reaction to spray used at home. We called MSHA 800 phone number, were told to complete this form. Driver is fine, was back at work following day.

EE HAD COMPLAINED OF HEARTBURN BUT DECLARED IT WOULD PASS. COWORKER SAW HIM EXIT THE LOADER AND HEAD TOWARD HIS CAR. COWORKER RETURNED ABOUT 3 MINUTES LATER, AND FOUND EE FACE DOWN ON THE GROU ND. TWO PEOPLE PERFORMED CPR UNTI EMS ARRIVED.
