Employee slipped on the boat when grabbing the rail and injured his shoulder. Employee did not report the injury until 12/4/2012.

lifted a battery resulting in an hernia

Pushing on bar, bar gave way, employee fell to floor. Ankle and knee were hurt.

Eye bolt broke loose from bucket, striking employee in leg. He was attempting to pull bucket together to weld a crack.

Dropped metal beam on finger.

Slag from welding fell into boot and onto foot, causing a burn.

Employee fell in boat because he was hit by a flying fish . Possible cartilage tear in right shoulder.

This morning 1/20/09 the employee informed us that yesterday he was lifting a bottle of Compress oxygen. This morning he told us of the accident. We put a heat pack on his neck & sent him to Concentra Medical. Concentra said he strained his neck and they are going to do 3 or 4 sessions of physical therapy on him.

Picked up jon boat and put in lake. Lumbar strain.

Slipped on stairwell and fell on left hip.

Cut right index finger on cable.

Slipped on stairs, falling on buttocks.

Dropped 6"" metal pipe on right foot - bruised foot.

Lifting a piece of pipe pulled musel in back

Tooth from loader bucket fell on right foot.

DOCKING WORK BARGE AND CAUGHT LEFT HAND IN HOOK ON THE CHAIN.

WORKING ON SHAKER AND SCRATCHED HAND, TREATED AS FIRST AID ON 10/25/04. BECAME INFECTED AND WENT TO DOCTOR 11/04/04. DOCTOR CLEANED AND PRESCRIBED ANTIBIOTICS.

Slipped on deck hand hit piece of sharp metal cutting hand. (hand was sutured-by mis mail 10/22/04 #5).

PULLING ON ROPE ON RIGHT TAIL LINE, HIS FEET SLIPPED AND HE FELL. BACK AND NECK WERE HURT. LAST DAY WORKED WAS 7/20/04.

A rock came off of conveyer belt, hit catwalk, bounced and hit employee in the back.

KNEE HIT ANGLE IRON ON SHAKER, LEFT KNEE CONTUSION.

KNEE HIT BOOM POLE.

CHAIN CAME LOOSE AND HIT HIS HAND.

EE LIFTED HEAVY BLOCK, EXPERIENCED LUMBAR STRAIN.

EE ON GROUND DIRECTED LOADER OPERATOR TO PUT BUCKET DOWN. EE DID NOT MOVE & BUCKET HIT HIS FOOT.

STEPPING ONTO WORK BARGE, SLIPPED, FELL, BRUISED SHOULDER AND RIBS.

MOUNTING PIPE, SLIPPED, FELL ON RIGHT HAND.
