This morning 1/20/09 the employee informed us that yesterday he was lifting a bottle of Compress oxygen. This morning he told us of the accident. We put a heat pack on his neck & sent him to Concentra Medical. Concentra said he strained his neck and they are going to do 3 or 4 sessions of physical therapy on him.

PULLING ON ROPE ON RIGHT TAIL LINE, HIS FEET SLIPPED AND HE FELL. BACK AND NECK WERE HURT. LAST DAY WORKED WAS 7/20/04.
A rock came off of conveyer belt, hit catwalk, bounced and hit employee in the back.
