lifted a battery resulting in an hernia
Pushing on bar, bar gave way, employee fell to floor. Ankle and knee were hurt.

Dropped metal beam on finger.
Slag from welding fell into boot and onto foot, causing a burn.

Employee fell in boat because he was hit by a flying fish . Possible cartilage tear in right shoulder.
This morning 1/20/09 the employee informed us that yesterday he was lifting a bottle of Compress oxygen. This morning he told us of the accident. We put a heat pack on his neck & sent him to Concentra Medical. Concentra said he strained his neck and they are going to do 3 or 4 sessions of physical therapy on him.

PULLING ON ROPE ON RIGHT TAIL LINE, HIS FEET SLIPPED AND HE FELL. BACK AND NECK WERE HURT. LAST DAY WORKED WAS 7/20/04.
