Employee was welding on 02/11/2026 and felt a little splinter of metal bothering EE's finger but initially did not seek treatment. The following day EE decided to seek treatment as it was still bothering EE's finger.

Employee was walking towards the tumbler and slipped on some ice and struck EE's right knee on the ground. EE was treated at an urgent care, returned to work with restrictions for 5 days.

Employee was operating a cutoff machine and was cutting stone that was 4 in. EE was holding the stone so that it would not move and was not paying attention to the machine. Saw lacerated right index finger.

Employee noticed a rash on abdomen on Friday the 29th and reported it to supervisor on Monday, August 1st as a rash or burn from being in the sun. We sent EE to the clinic and it was determined to be hives. We are still waiting for the clinic physician to notify us whether the hives were a result of heat from the sun or another source. EE was given RX topical cream.

Although employee was notified that he is required to wear steel toed boots at all times he removed them because they were bothering him. He hit his toe on a conduit tube embedded in the ground. He did not seek medical attention until 12/15, first aid was done, no RX was dispensed. Returned for further med. attention on 12/24 and RX meds were provided.

No unsafe conditions. Employee was throwing a rock into a metal bin and part of the rock slit off. This cause his ringer to become caught between the rock and the bin when he threw it.

Due to continued lifting of heavy material the employee's back started hurting, then he noticed a hernia on October 28th in his groin area. The condition was reported as workers comp to the insurance co, and was denied. However, as a result of a benefit dispute, the initial decision was reversed on 5/30/13 and is now being considered a work related injury.

While chiseling stone a small piece of rock flew up under his safety glasses and into his left eye. This did not bother him at the time and he was initially unaware until he got home that night and his eye was irritated. He notified the employer the next day, 10/5 and was sent to a clinic where he was given eye drops. Returned to work immediately with no further issues.

Was stacking stone on a pallet and caught his thumb in between two pieces of stone which resulted in a contusion of his left thumb.

He was measuring a rock on the chopper when someone pushed other rocks over causing his right hand to be caught between them. This resulted in a fracture to right wrist and bruised palm.

He was placing a piece of stone on the radial arm platform and the stone broke. One piece hit his knee bruising it and making it sore.

He was putting pieces of rock on top of a conveyor for the chopper and misjudged the placement of his finger and jammed between the rock and the metal siding of the device. He broke his pinky and had a laceration on the top of the finger which required stitches. He has not returned to work.

He was on top of the hyster loader cleaning it out and he slipped and fell landing on his feet. This resulted in a possible fracture in his right heel.
