EMPLOYEE WAS HELPING ANOTHER EMPLOYEE DRIVE A WEDGE USING A HAMMER. THE HANDLE OF THE HAMMER BROKE AND THE HAMMER HEAD FLEW OFF GLAZING THE EMPLOYEE'S JAW. THE HAMMER HIT HIS TEETH AND BROKE T HE SECOND TOOTH FROM THE CENTER ON THE RIGHT SIDE OF HIS MOUTH.
