EMPLOYEE DROPPED JACK HAMMER ON TOP OF A ROCK, HIS FINGER WAS CAUGHT BETWEEN JACK HAMMER AND ROCK CUTTING HIS FINGER.
EMPLOYEE WAS USING A TORCH TO CUT A PIECE OF METAL LAYING ON GRANITE ROCK. A PIECE OF HOT GRANITE ROCK FLEW UP UNDER SAFETY GLASSES HITTING RIGHT EYE.
EMPLOYEE WAS SPLITTING A ROCK. WHEN IT SPLIT A PIECE FELL CAUSING EMPLOYEE TO LOOSE HIS BALANCE AND WHEN HE CAUGHT HIMSELF WITH HIS LEFT ARM IT CAUSED THE SHOULDER TO SEPARATE.

EMPLOYEE WAS HELPING ANOTHER EMPLOYEE DRIVE A WEDGE USING A HAMMER. THE HANDLE OF THE HAMMER BROKE AND THE HAMMER HEAD FLEW OFF GLAZING THE EMPLOYEE'S JAW. THE HAMMER HIT HIS TEETH AND BROKE T HE SECOND TOOTH FROM THE CENTER ON THE RIGHT SIDE OF HIS MOUTH.
