Bodily reaction, (Not Elsewhere Classified)

Loader operator felt back pain, it was all of a sudden. They did not fall nor slip or do anything out of the ordinary. The pain grew more intense and they went to the doctor that evening.

Supervisor was working on the dozer and hit EE's left thumb with a rachet tool. It caused the finger to swell and hurt.

Fall from machine

Employee was operating loader and hauling materials. They got down at the shop but missed stepped and fell from the steps of the loader. It was a clear morning and no signals of bad weather conditions.

Employee was going up the stairs to the scale house office and EE missed EE's step and twisted EE's back.

Fall onto or against objects

During the morning of 12/10/21, EE went up the 938H equipment to do maintenance and lube valves. The fog was dense and the equipment was wet. EE slipped and fell hitting self against the equipment bucket. EE received injury to EE's back. No swelling, no bruising but EE did have pain. EE went to Urgent Care for treatment and checkup.

Worker has been under doctor's care since 12/23/2019 with no restrictions. On 01/03/2020 worker's doctor has now applied work restrictions. No bending, climbing, no operating heavy equipment, no lifting more than 25 lbs. Back related injury received when dozer allegedly ran into the 740 truck.

Fall from ladders

Employee changing oil in Euclid. Employee walking behind Supervisor coming out of storage bldg. with filters in hand. Employee jumped 2 1/2 foot off the ladder to the ground. Refused to go to doctor. 8/11 employee call supervisor complaining of back pain. Went to doctor-gave restrictions for job. No light duty available at time. Employee called in 8/18/14 and resigned position.

EMPLOYEE WAS CHANGING SCREEN ON ROCK CRUSHER, WHILE PUTTING IN BOLT, A PIECE OF METAL SHEARING CAUGHT THE RIGHT MIDDLE FINGER CAUSING A SMALL PIECE OF METAL TO BE IMBEDDED IN THE FINGER. THE F INGER INFECTED, EMPLOYEE WAS HOSPITALIZED FOR ANTIBIOTIC TREATMENTS SINCE THE INFECTION WAS COMPLICATED BY THE EMPLOYEE BEING DIABETIC.
