Operator was loading material in the crusher. A large rock became stuck, when ee climbed the stairs to inspect the problem ee's foot slipped and ee fell. EE hit ee's side on the rail and fell to the ground.

Slipped while climbing up in excavator and fell onto rocks below injuring ribs on left side.

EE was working on light tower. Two pipes were stuck together, while pulling and twisting to get them apart he hurt his back.

Employee picked up a rock that was in the mine traffic area and strained his back.

Employee became disoriented and was walking along mine road. He was taken to local hospital, then transferred to Austin for further testing. The hospital has still not drawn any conclusive reason for his being disoriented. Perhaps medication issue. He is being seen by a neurologist and his pcp. He has not returned to work as of this time.

Cause-Heavy smoker. Heart Attack-Stint put in this date and plan to do by-pass surgery in (3) weeks.

EE says that he lost his footig on the track of the excavator and caught himself with the hand rail. Reports his shoulder is sore and hurting, so he is seeing a doctor.

Injured was checking hydraulic oil & when climbing up, shoe hung on step causing him to fall back hitting on cylinder which blocked his fell but caused him to pull right side of ribs & pull on his back.

Employee stepping down from crusher and twisted ankle.

Employee not wearing hard hat as required. Employee given written reprimand and suspension for safety violation.

Employee jumped down from a 2 foot high material chute and sprained ankle.

Employee stepping down from excavator stepped in rocks and fell, twisting right knee.

EE RAISED WINDSHEILD IN UP POSITION TO BE ABLE TO SEE BETTER. WINDSHIELD HELD FOR A SHORT TIME THEN SUDDENLY FELL ALONG WITH FRAME STRIKING EE ON HEAD CAUSING MINOR INJURY TO HIS HEAD & NECK.

EE WAS STANDING ON BACK OF MAINTENANCE UTILITY TRUCK OBSERVING WORK BEING DONE ON GEAR BOX ON CONVEYOR BELT FOR SCREENING PLANT. EE STEPPED BACK WITHOUT LOOKING & STEPPED OFF TRUCK FALLING APPROX. 4' TO THE GROUND. BRUISING ON LEFT SIDE & RIBS FROM IMPACT OF FALL.
