Mechanic was adjusting blow bar and small piece of metal flew off blow bar and struck Mechanic in chest.

EE was cleaning under the shaker when EE noticed one of the wipers on a conveyor belt was damaged, EE proceeded to make a new one. As EE was making the wiper, EE was hitting the hole punch with the hammer. EE swung the hammer really hard and it slipped and struck EE's hand. EE went to the break room, removed their glove, called EE at the crusher, EE called another EE.

Employee was delivering from one mine site to another and was involved in a MVA on public road

Employee was washing trucks on mine site with high pressure water and hose and while nozzle was on, employee let go of handle and lost control of the wand, as the wand swung the water and pressure struck the employee's left eye lid, causing a laceration.

Fall from machine

Miner was washing haul truck while standing on the trucks access ladder. As employee opened the spray nozzle, the pressure caused the miner to become unstable and fall from the ladder approx. 4-5 feet. Employee fell onto left hand causing a fracture to left middle finger.

Contact with hot objects or substances

Mechanic was removing a lower leaf spring bushing and was using a cutting torch to heat and melt the inner rubber to simplify the removal of a metal the metal pin. As mechanic applied heat to the bushing melted rubber spat put of the metal sleeve onto the inner side of his right elbow and forearm.

Struck against stationary object

Miner was performing crusher maintenance and as employee was removing crusher fines from chute, employee struck his left middle finger against a metal guard causing a distal fracture

Miner was assisting maintenance in the removal of crusher ""Blow Bar"", as miner was removing Blow Bar locking assembly (rod) with a jack hammer, the rotor shifted and caused the jack hammer and positioning sleeve to shift, as a result the miners' 3rd digit (middle finger) was caught in between the sleeve and rotor. Miner suffered loss of nail, flesh and tissue.

Mine haul truck driver attempted to free stuck conveyor belt and was using pry bar to free conveyor as conveyor motioned it caused miner to let go of pry bar and get his right 4th finger caught in between pry bar and conveyor guarding.

Crusher Conveyor feed belt immediately stopped working, so operator was observing crusher motor to evaluate the condition and climbed on a portable crusher deflated tire to inspect closer. Once miner was standing on tire it slipped causing miner to fall onto rock pile below. Miner received bruising to lower back left side and a sprained finger.

Mechanic was removing a bearing to a crusher motor on yard/shop site of mine property. Employee struck bearing with ball ping hammer and a piece (chip) of the hammer struck the employee and was embedded into the left lower arm above the wrist of the employee, employee was wearing all proper ppe while performing his task. EE was taken to clinic for foreign body removal.

Changing the blow bars on the crusher when the pedestal for the blow bar came loose and hit the handrail. The handrail hit the EE. EE was hit on the right side of his stomach and top of his right hip bone. He suffered bruising and a minor abrasion to his stomach area.

Working on trailer, air bag released and air blast pushed employee's head against the ground. He was on the ground to the left of the air bags. No visable injury; taken to hospital as a precaution.
