November 11, 2005 TX · Metal/Non-Metal haul/off road/coal/ore/pit/quarry/rock/rubber tire truck driver, transportation truck driver SLIP OR FALL OF PERSON

EE WAS GREASING A CAT 769 HAUL TRUCK. THE EE WAS STANDING ON THE FRAME OF THE TRUCK USING A GREASE HOSE ON REEL. THE GREASE RELEASED ITS LOCK AND JERKED THE EE CAUSING HIM TO FALL FROM TRUCK.
